术中超声与术中胆管造影诊断胆总管结石的对比性研究

目的探讨术中超声检查在诊断胆总管结石方面的临床应用价值。方法对2002年1月~2005年12月我院收治的121例胆石病病人的临床资料进行前瞻性分析。结果本组确诊为胆总管结石者108例。术中超声正确诊断者106例,假阴性2例,无假阳性;术中造影正确诊断者105例,假阳性和假阴性各3例。两种检查方法在敏感性、特异性和准确率上无显著性差异(P>0.05),但在检查所需时间上有显著性差异(P<0.05)。术中超声检查无失败病例;术中胆管造影有5例插管失败。结论术中超声具有无创、快速、省时、可重复、成功率高等优点,是术中胆管造影可靠的替代方法。

Comparative study of intraoperative ultrasonography vs cholangiography in diagnosis of choledocholithiasis

Objective To evaluate the role of intraoperative ultrasonography(IOUS)in diagnosis of choledocholithiasis.Methods 121 cases of cholelithiasis from Jan. 2002 to Dec. 2005 in our hospital were prospectively analyzed.Results 108 of 121 were finally diagnosed as having choledocholithiasis.By IOUS,106 cases were diagnosed correctly,2 cases were false negative,and no cases false positive.By IOC,105 cases were diagnosed correctly,3 cases were false negative and 3 false positive respectively.There was no significant difference between IOUS and IOC in sensibility,specificity and accuracy,except time consuming.No failure occurred in IOUS,but 5 failures in IOC.Conclusion IOUS is noninvasive,fast,repeatable,and highly successful,could reliably substitute for intraoperative cholangiography.
