流产术后近期卵巢功能恢复状况的比较观察

目的:观察药物流产、手术流产两种方式终止早期妊娠后近期卵巢功能恢复状况。方法:应用基础体温测量、B超监测、宫颈粘液观察、孕酮水平测定四种方法比较65例妇女药物和手术流产后卵巢功能恢复状况。结果:药物流产组84.85％(28/33)的妇女和手术流产组71.88％(23/32)的妇女在第一个月经周期可恢复排卵,其中20天以内排卵的妇女药物流产组占60.71％(17/28),手术流产组占30.43％(7/23),两组间有显著性差异(P<0.01)。药物流产组40％(12/30)有类似黄体功能不足的表现,手术流产组为15.38％(4/26),两组间有显著差异(P<0.05)。药物流产组黄体期平均孕酮值也较手术流产组低(P<0.05)。结论:虽然药物流产组流产后出血时间明显长于手术流产组,但两组大多数妇女在流产后很快恢复排卵,药物流产妇女在20天内排卵者较多,但表现为黄体功能不足率较高。

Resumption of Ovarian Function Following Medical and Surgical Abortion

Objective: To study the resumption of ovarian function after medical or surgical abortion in the first trimester. Methods: 65 patients underwent medical or surgical abortion in the first trimester were followed with basal body temperature, B ultrasonic, cervical mucus and serum progesterone level before the first menstrual bleeding. Results: 71. 88% (23/32) cases of surgical abortion ovulated in the first menstrual cycle,and 30. 43% (7/23) ovulated within 20 days after abortion. 84. 85% (28/33) cases of medical abortion ovulated in the first menstrual cycle, and 60. 71% (17/28) ovulated within 20 days after abortion. 15. 38% (4/26) cases expressed luteal phase deficiency after surgical abortion, 40% (12/30) patients expressed luteal phase deficiency after medical abortion. The mean serum progesterone level in luteal phase after medical abortion was lower than that after surgical abortion (P < 0. 05). Conclusion: The rate of ovulation was high in both groups. After medical abortion, there were more large amounts cases who had ovulated before 20th day, but had expressed luteal phase deficiency.
