HRCT在涂阴活动性肺结核诊断中的应用价值

目的：探讨HRCT在痰涂片阴性活动性肺结核诊断中的应用价值。方法：回顾性分析267例痰涂片阴性疑似活动性肺结核患者的HRCT资料,被证实涂阴活动性肺结核患者173例作为涂阴结核组,其他肺疾病患者94例作为对照组,随机选择100例痰涂片阳性肺结核患者组成涂阳结核组,对以上各组HRCT表现进行统计学分析。结果：结节（<10mm）、“树芽征”、肺实变、支气管损害、病灶分布（上叶尖段、后段,下叶背段）等征象与涂阴活动性肺结核有显著联系;结节、“树芽征”、肺实变等HRCT征象,在痰涂片阴性、阳性患者间差异无统计学意义。空洞、胸腔积液、纵膈淋巴结增大等HRCT征象,在涂阴活动性肺结核HRCT中较少出现,在痰涂片阴性、阳性患者间有显著性差异。结论：HRCT对诊断及鉴别涂阴活动性肺结核有重要意义。

The value of sputum smear negative active pulmonary tuberculosis by HRCT

Objective:To investigate HRCT in sputum smear-negative active TB diagnosis value.Methods:A retrospective analysis of 267 cases of smear-negative patients with suspected active tuberculosis HRCT data; been confirmed smear-negative patients with 173 cases of active tuberculosis as the smear-negative tuberculosis group,the other lung disease 94 patients as a control group,Randomly selected 100 cases of sputum smear-positive pulmonary tuberculosis patients as smear-positive tuberculosis group,the HRCT performances were statistically analyzed.Results:nodules,"tree-in-bud appearance",pulmonary consolidation,bronchial damage,damage is mainly distributed (on the tip posterior segment, under the dorsal segment) have significant contact with smear-negative active tuberculosis.nodules,"tree-in-bud appearance",pulmonary consolidation have no statistical difference between the patients with sputum smear negative and positive significance.cavity, pleural effusion, mediastinal lymph nodes enlargement are less frequent in smear-negative active tuberculosis,there are significant differences between sputum smear negative and positive patients.Conclusion:HRCT in the diagnosis and differential smear-negative active tuberculosis has important significance.
