温阳补肾方对甲减致性腺功能减退患者血清甲状腺激素及性激素水平的影响

目的观察温阳补肾方治疗甲状腺功能(甲减)导致的性腺功能减退的疗效及对患者血清甲状腺激素及性激素水平的影响。方法将2019年1月—2020年1月沈阳市第五人民医院诊治的76例甲状腺功能减退导致的性腺功能减退患者随机分为2组,对照组38例给予左甲状腺素钠片口服治疗,观察组38例在对照组治疗基础上给予温阳补肾方口服治疗,2组均连续治疗12周。对比2组治疗前后基础代谢率(BMR)、血脂指标总胆固醇(TC)、三酰甘油(TG)]、甲状腺功能指标血清总三碘甲腺原氨酸(TT3)、总甲状腺素(TT4)、游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)]及血清性激素指标雌二醇(E2)、睾酮(T)、孕激素(P)、促卵泡生成素(FSH)、促黄体生成素(LH)],统计2组临床疗效及不良反应发生情况。结果2组治疗后BMR及血清TT3、TT4、FT3、FT4、E2、T、P、FSH、LH水平均明显升高(P均<0.05),且观察组明显高于对照组(P均<0.05);2组治疗后TC、TG、TSH水平均明显降低(P均<0.05),且观察组均明显低于对照组(P<0.05);观察组总有效率为89.5%(34/38),对照组为68.4%(26/38),观察组明显高于对照组(χ²=5.067,P<0.05);2组患者治疗期间均未出现明显不良反应。结论温阳补肾方能够调节甲状腺功能减退导致的性腺功能减退患者甲状腺激素和性激素变化,有助于提高患者甲状腺功能和性腺功能,改善预后。

Effect of Wenyang Bushen Decoction on serum thyroid hormone and sex hormone in patients with hypogonadism caused by hypothyroidism

Objective It is to observe the curative effect of Wenyang Bushen Decoction on hypogonadism caused by hypothyroidism and its effect on the levels of serum thyroid hormone and sex hormone in the patients.Methods Seventy-six patients with hypogonadism caused by hypothyroidism diagnosed and treated in the Fifth People’s Hospital of Shenyang from January 2019 to January 2020 were randomly divided into 2 groups.38 cases in the control group were given levothyroxine sodium tablets orally.38 cases in the observation were given Wenyang Bushen Decoction orally based on the control group.Both groups were treated for 12 consecutive weeks.The basal metabolic rate(BMR),blood lipid indexestotal cholesterol(TC),triacylglycerol(TG)],thyroid function indexesserum total triiodothyronine(TT3),total thyroxine(TT4),free triiodothyronine(FT3),free thyroxine(FT4),thyroid-stimulating hormone(TSH)]and serum sex hormone indicatorsestradiol(E2),testosterone(T),progesterone(P),follicle stimulating hormone(FSH),luteinizing hormone(LH)]before and after treatment were compared between the two groups,the clinical efficacy was evaluated and adverse reactions were counted in the 2 groups.Results After treatment,BMR and the levels of serum TT3,TT4,FT3,FT4,E2,T,P,FSH,LH in the two groups were significantly increased(all P<0.05),and these indexes in the observation group were significantly higher than those in the control group(all P<0.05).After treatment,the levels of TC,TG and TSH in the two groups were significantly reduced(P<0.05),and theses indexes in the observation group were significantly lower than those in the control group(all P<0.05).The total effective rate of the observation group was 89.5%(34/38),that of the control group was 68.4%(26/38),the observation group was significantly higher than the control group(P<0.05);there were no obvious adverse reactions in the two groups during treatment.Conclusion Wenyang Bushen Decoction can regulate the changes of thyroid hormone and sex hormones in patients with hypogonadism caused by hypothyroidism,which can help improve the thyroid function and gonadal function and improve the prognosis.
