不同浓度的安氟醚对肝肾功能的影响

摘    要:目的 观察不同浓度的安氟醚对部分血生化指标的作用,探讨该药对肝肾功能的影响。方法24例 ASAⅠ~Ⅱ级全麻手术患者随机分为Ⅰ、Ⅱ、Ⅲ组,均为8例,分别吸入1%、2%、3%安氟醚。于吸入安氟醚前(T_0)、吸入安氟醚2h(T_1)、4h(T_2)、24h(T_3)和7d(T_4)取静脉血,测天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、血糖(GLu)、总胆固醇(CHOL)和总胆红素(TBIL)、血肌酐(CRE)及尿素氮(BUN)等生化指标。结果 Ⅰ组吸入安氟醚前后各生化指标无明显变化。其他两组吸入安氟醚后AST、ALT、GLu、CHOL、TBILL有不同程度升高(P<0.01或P<0.05),吸入浓度越大上述指标升高越显著,7d后回降至正常。三组吸入安氟醚前后CRE和BUN均无明显变化。结论 吸入较高浓度安氟醚可使AST、ALT、GLu、CHOL和TBIL出现不同程度的变化,而CRE、BUN无明显变化。吸入较高浓度对肝功能可能有一定损害。

Abstract:Objective By studying the effects of enflurance in different concentration on partial blood biochemical parameters to evaluate the influence of this drug on hepatic and renal functions. Methods Twenty-four ASA Ⅰ - Ⅱ grade patients undergoing elective surgery under general anesthesia with tracheal intubation were divided into three groups random: group Ⅰ (8), group Ⅱ (8) and group Ⅲ (8). Enflurane in 1% ,2% ,3% concentration was inhaled respectively. The vein blood was obtained before enflurane inhalation (T0),at 2h (T1) and 4h (T2) in enflurane inhalation, at 24h (T3) and 7d (T4) after enflurane inhalation. The plasma biochemical parameters of aspartate aminotransferase(AST), alanine aminotransferase (ALT), glucose (Glu), total cholesterol(CHOL), total bilirubin (TBIL), creatinine (CRE) and urea nitrogen (BUN) were determined. Results In group Ⅰ , these biochemical parameters had no significant difference before and after enflurane inhalation. However, AST, ALT, Glu, CHOL and TBIL raised in some degree after enflurane inhalation in the other two groups. The higher inhalated enflurane concentration, the higher these biochemical parameters increased. These inereased biochemical parameters returned to normalities late seven days after enflurane inhalation. Also, CRE and BUN had no significant change before and after enflurane inhalation in all the 3 groups. Conclusion The high concentration enflurane inhalation may increase the levels of AST,ALT,Glu,CHOL and TBI, whereas this doesn't happen on CRE and BUN. The inhalation of higher concentration enflurane would damage liver function at certain content.
