摘    要:目的:探讨牵引按摩对颈源性头痛(CH)患者脑血管机能障碍改善的机理。方法:用经颅多谱勒超声(TCD)检测125例有颈椎曲度异常或颈椎退行性改变的颈源性头痛患者牵引按摩前后与正常对照组90例的9条颅内血管(双侧大脑前、中、后动脉、椎动脉及基底动脉),并对TCD结果做统计学分析。结果:1.颈源性头痛组(125例)牵引按摩治疗后,大脑中、前动脉的平均血流速度(Vm),较治疗前降低,其值更接近于正常对照组(P〈0.05或P〈0.01)。2.治疗后颅内成对动脉的Vm差异缩小,其代表值-非对称指数(AI)的异常率以及均值显著低于治疗前,与正常对照组更接近(P〈0.01)。结论:牵引按摩治疗可双向调整颅内动脉的异常血流速度增高和减低,从而改善颈源性头痛患者的脑血管机能紊乱。

Abstract:Objective: To study the possible mechanism of traction and massage to improve cerebral vessels function in cervicogenic headache (CH). Method: Transcranial doppler sonography (TCD) was performed to explore the intracranial segments of internal carotid artery (cerebral anterior, middle and posterior arteries), bilateral vertebral arteries, and basilar artery in 125 CH cases and 90 healthy adults before and after physical treatment. Result:1. In the total tested 125 cases of CH group, after treatment, the mean velocity of blood flow (Vm) of cerebral anterior and middle arteries decreased to normal value of control group(P<0.05). 2. The abnormality ratio and average values of the antisymmetry index(AI)which represents the intracranial paired arteries Vm disparity were significantly decreased after treatment in CH group compared with those before treatment and the control group (P<0.01). Conclusion: The treatment of traction and massage possesses bidirectional regulating effects on the cerebral blood circulation, there by reduces the headache attacks in CH patients.
