摘    要:目的 了解尿肺炎链球菌抗原用于辅助诊断肺炎链球菌感染的临床意义。方法 收集2000年9月~2001年2月上海儿科医院0~3岁下呼吸道感染的患儿病例,对每位患儿同时采集尿标本和痰标本,用免疫层析法测定尿肺炎链球菌抗原,对痰标本进行分离培养与鉴定。结果 210例患儿中,尿肺炎链球菌抗原阳性率为33.8%(71/210),痰标本阳性率为32.9%(69/210)。肺炎链球菌阳性组尿肺炎链球菌抗原阳性率(79.7%)显著高于阴性组(11.3%)(P

Abstract:Objective Streptococcus pneumoniae has long been one of the most important bacterial pathogen causing acute lower respiratory tract infection (ALRI) . Currently the rapid, sensitive and specific tests are not available which can be used for etio-logic diagnosis of ALRI. The present study was designed to determine the effect of pneumococcal antigen in urine (UPA) on clinical diagnosis of pneumococcal infections. Methods The subjects involved 210 patients aged 0 - 3 years old with clinical and roentgeno-graphic evidence of acute lower respiratory tract infection attending Shanghai Children's Hospital from Sep 2000 to Feb 2001. For individual patient, urine specimen was collected for detecting UPA by Immunochromatographic test (ICT) , and sputum specimen for culture. Steptococcus pneumoniae (Sp) was identified by optochin sensitivity and bile solubility. Results Of 210 patients the detection rates of UPA were 33.8% .The isolation rates of Sp from sputum specimens were 32.9%. The rates of UPA in positive group were apparently higher than that of negative group ( P < 0.01) . The sensitivity, specificity and accuracy of UPA by ICT were 85.2%, 88.7% and 87.7% respectively. Conclusions ICT for detection of UPA, a relative sensitive and specific test, is useful for aiding clinical diagnosis of pneumococcal infections.
