曲安奈德在复杂白内障手术中的应用

目的探讨复杂白内障手术中应用曲安奈德对预防术后并发症的临床效果。方法88例121眼复杂类型的白内障，包括外伤性、葡萄膜炎并发性及抗青光眼术后白内障眼。患者采用完全随机设计法分为2组：观察组（43例58眼）白内障术中前房内注射曲安奈德注射液2mg；对照组（45例63眼）术中未注入曲安奈德。随访3—24个月，观察2组术后并发症及视力恢复情况。结果观察组术后前房炎症反应较对照组轻，2组分别为1．72％和20．63％，差异有统计学意义（P〈0．05）。观察组虹膜后粘连和后囊混浊发生率低于对照组，2组分别为6．90％、5．17％和23．81％、19．05％，差异有统计学意义（P〈0．05）。观察组术后视力好于对照组，视力〉0．5者2组分别为84．48％和65．08％，差异有统计学意义（P〈0．05）。2组均未发生角膜内皮功能失代偿和持续性高眼压。结论复杂白内障手术中前房注射曲安奈德能够有效地预防术后并发症的发生，术后视力恢复好，安全方便。

Triamcinolone acetonide in complex cataract surgery

Objective To investigate the therapeutic effect of triamcinolone acetonide(TA) for decreasing postoperative complications in complex cataract surgery. Methods Eighty-eight cases (121 eyes) of complex cataract were collected and divided into two groups: experimental group (43 cases, 58 eyes) and control group (45cases, 63 eyes), including traumatic cataract, complicated cataract with uveifis and complicated cataract after anti-glaucoma surgery. During the cataract operation,TA 2rag was injected into the anterior chamber while the control group had no TA injection in experimental group. The follow-up time was between 3 and 24 months. Postoperative complications and visual acuity were observed. Results Anterior chamber inflammatory reaction in experimental group(1.73%) was less(P<0. 05) than that in the control group(20.63%)The incidence of posterior synechia and posterior capsule opacification in experimental group(6.90%, 5.17%) was lower (P<0.05) than that in control group (23.81% vs 19.05%). The visual acuity(above 0.5) in experimental group(84.48%) was better(P<0.05) than that in control group (65.08%). None of these eyes bad corneal endothelial decompensation and durative high intraoeular pressure. Conclusion Injecting TA into anterior chamber in complex cataract surgery is a safe and convenient method. It proves to prevent postoperative complications effectively and provide a better visual acuity.
