多囊卵巢综合征中胰岛素抵抗与高雄激素血症

目的：探讨多囊卵巢综合征（ＰＣＯＳ）者胰岛素抵抗与高雄激素血症的关系。方法：测定４０例ＰＣＯＳ患者和２０ 例排卵正常、不孕妇女的血清雄激素、性激素结合球蛋白（ＳＨＢＧ）含量，并观察口服葡萄糖耐量试验（ＯＧＴＴ）时血糖、胰岛素变化。结果：ＰＣＯＳ组雄激素水平显著升高（Ｐ＜０．０５），ＳＨＢＧ显著降低（Ｐ＜ ０．００１），胰岛素敏感性显著降低（Ｐ＜ ０．０１）；且胰岛素与雄激素呈正相关，而与ＳＨＢＧ呈负相关。结论：高胰岛素血症和高雄激素血症可能是ＰＣＯＳ无排卵机制之一。

Insulin Resistance and Hyperandrogenemia in Polycystic Ovarian Syndrome

Objective:To investigate the relationship between insulin resistance and hyperandrogenemia in patients with polycystic ovarian syndrome(PCOS).Methods:The serum androgen and sex hormone binding globulin(SHBG) were measured in 40 PCOS patients and 20 women with ovulatory infertility.An oral glucose tolerance test(OGTT) was performed,the responses of glucose and insulin during OGTT were measured in all subjects.Results:Serum androgen were significantly elevated in patients with PCOS ( P <0.05).The level of SHBG was significantly lower in PCOS women compared with controls( P <0.001).Insulin sensitivity in PCOS group was significantly lower than that in controls( P <0.001).A positive correlation existed between insulin and androgen levels.There was a negative correlation between insulin and SHBG levels.Conclusion:Insulin resistance and hyperandrogenemia were two related features of PCOS,which may be one of the mechanisms of anovulatory in PCOS.
