腹腔镜胆囊切除术后并发症的防治体会

目的探讨LC术后并发症的原因,术中预防及术后处理方法。方法回顾分析1997年6月至2005年9月1206例LC患者临床资料。结果1206例中发生并发症35例占2.90%。其中胆管损伤1例;胆漏16例;出血3例;胆绞痛12例(其中包括胆囊床积液5例);脐部刺口大网膜嵌顿1例;麻痹性肠梗阻2例,均治愈出院。结论LC术后尽管有个别并发症,但术前严格把握手术适应证,术中视病人具体情况仔细操作灵活选择手术方式。术后严密观察病情变化,及早发现并发症,正确处理。降低LC手术并发症,提高手术安全性。

PREVENTION AND TREATMENT OF COMPLICATIONS AFTER LAPAROSCOPICCHOLECYSTECTOMY:A REPORT OF 1206 CASES

Objective To study the complicatiou reasons after laproscopic cholecystectomy(LC)and summary the measures of prevention and treatment.Methods The Clinic Data of 1206 patients performed LC between June 1997 and september 2005 was analyzed retrospectively.Results Among all the case,The complication rate was 2.9%%(35 cases patients).The complication included extrahepatic bile ducts injury(1 case),bleeding(3 cases),bile leakage(16 cases).cholecystitis pain(12 case),Include fossa of gallbladder amassing liquid(5 cases).paralytic ileus(2 case).Omentum majus set in trocar sites(1 case).All cases have been cured and discharged.Conclusion Even though LC has a little complications.Each patients is cotrolled surgical adaptation strictly.operated carefully.When the situation is difficult,the LC should be converted to OC timely.Once the serious complication is presented.it is important to keep intra-abdominal drainage unobstruct and observe monitor the vital signs.Finding the complicatiou timely and treated correctly.LC is regared effective minimally invasive surgical thechnology.
