全肺灌洗对肺纤维化兔动脉血气的影响

摘    要:目的观察全肺灌洗对肺纤维化兔动脉血气的影响。方法将25只健康新西兰大白兔按随机数字表法分为3组,对照组(n=5)、模型组(n=5)及实验组(n=15)。又将实验组分为7 d组1、4 d组及21 d组3个亚组,每组各5只。实验第1天,对照组经气管内注入生理盐水2 mL,模型组和实验组经气管内一次性注入平阳霉素8 mg.kg-1(溶于生理盐水2 mL中)成功制备肺纤维化动物模型。7 d组1、4 d组及21 d组分别在动物模型制备后的第7、14、21天行全肺灌洗治疗。在实验第1天和第5周末行动脉血气分析,观察各组pH和动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)的变化。结果实验第1天各组pH、PaO2及PaCO2变化比较差异无统计学意义(P>0.05)。第5周末,模型组的pH低于对照组;对照组PaO2高于其他各组(P<0.05或P<0.01);7 d组和14 d组的PaO2高于模型组(P<0.05或P<0.01);7 d组PaO2高于21 d组(P<0.05)。结论早期全肺灌洗可以显著提高肺纤维化兔的PaO2。

Abstract:Objective To investigate the effect of whole lung lavage on arterial blood gases in rabbits with pulmonary fibrosis.Methods Twenty-five healthy New Zealand White rabbits were randomized into three groups:control group(n=5),model group(n=5),and experimental group(n=15).Experimental group were further divided into three subgroups:7 days subgroup,14 days subgroup and 21 days subgroup,with 5 rabbits in each subgroup.Rabbit pulmonary fibrosis was induced by intratracheal injection with pingyangmycin(8 mg·kg-1).The control rabbits were given 2 mL normal saline only.Rabbits in the three subgroups were treated with whole lung lavage at 7,14 and 21 days after induction of pulmonary fibrosis,respectively.Arterial blood gas analysis was performed and the changes in pH,PaO2 and PaCO2 were observed on the first day and the fifth weekend of experiment.Results There were no significant differences in pH,PaO2 and PaCO2 among these groups on the first day of experiment(P0.05).On the fifth weekend,model group had lower pH and higher other groups PaO2 than control group(P0.05 or P0.01).PaO2 in the 7 days and 14 days subgroups was higher than that in model group(P0.05 or P0.01),and PaO2 in 7 days subgroup was higher than that in 21 days subgroup(P0.05).Conclusion Whole lung lavage can significantly increase PaO 2 in the early stage of pulmonary fibrosis in rabbits with pulmonary fibrosis.
