肥胖儿童2型糖尿病和高危者社区综合干预的效果分析

目的]探索肥胖儿童2型糖尿病的社区管理适宜模式。方法]2008年对深圳市宝安与光明新区5所中、小学肥胖儿童中筛查出的2型糖尿病和高危者建立档案，采用多种形式进行为期1年的社区综合干预。结果]198名儿童及家长对糖尿病相关知识、饮食控制、运动疗法、药物疗法、按时体检的知晓率，干预1年后均高于干预前（P〈0．01）；儿童空腹血糖、餐后2h血糖、糖化血红蛋白、总胆固醇和甘油三酯，干预1年后均明显低于干预前（P〈0．01）。结论]对肥胖儿童中的2型糖尿病患者和高危人群开展社区综合干预，对儿童2型糖尿病的预防和治疗有明显的效果。

Study on Effect of Comprehensive Intervention in Communities of Obese Children with Type 2 Diabetes and High-risk Persons

Objective]To explore the suitable management model for obese children with type 2 diabetes and high-risk persons by building the community comprehensive intervention system. Methods]Based on constructing the health documents of the obese children with type 2 diabetes and the high risk persons through screening in 5 middle and primary schools of Baoan district and Guangming new district of Shenzhen in 2008,carrying through a one-year comprehensive in- tervention experiment in communities. Results]The awareness rate of diabetes-related knowledge, diet. exercise pharmaceutical therapy,and periodic physical examination of the children with type 2 diabetes and high-risk persons was higher than that before the intervention（ P〈0.01） ;and the level of the indexes such as fasting blood-glucose,2 hours postprandial blood glucose, glycohemoglobin（HbAlc）, total cholesterol （TC） and triglyceride （TG） were all obviously lower than that before the intervention（ P 〈0.01）. Conclusion]It＇s helpful for prevention and treatment for type 2 diabetes through doing comprehensive communitylevel intervention for the obese children with type 2 diabetes and the high-risk persons.
