初复治肺结核病人免疫功能指标变化的研究

摘    要:目的]探讨肺结核病人的的免疫功能变化,以及相关指标在初复治、菌阴阳组之间是否存在差异。方法]采用流式细胞术检测外周血B细胞、T细胞亚群、NK细胞以及采用免疫透射比浊法测定IgG、IgA、IgM。结果]肺结核患者与正常人相比,CD4+T细胞、CD8+T细胞,CD4/CD8、IgG差异有统计学意义;初治菌阳组与正常对照组相比较,CD4+T细胞、CD4/CD8的差异有统计学意义;初复治对CD4+T细胞计数的影响有统计学意义。菌阴阳对各指标之间差异无统计学意义。结论]CD4+T细胞对肺结核病人的预后有重要的影响。CD8+T细胞及体液免疫在肺结核的细胞免疫功能中发挥重要作用。

Abstract:Objective] To analyze difference between tuberculosis patients and normal people in immunity,and investigate the two factors(initially-treatment,retreatment;sputum smear positive,sputum smear negative)whether affect lung tuberculosis patients'immunity or not.Methods]FCM was performed for analysis of B cells,T cells,and NK cells in peripheral whole blood of our cases.Immunity transmission turbidity was employed to detect IgA,IgG and IgM.Results]It had significant difference between tuberculosis patients and normal group in level of CD4+T cell,CD8+T cell,CD4/CD8 ratio(P0.05).meanwhile,CD4+T cell and CD4/CD8 ratio levelhad significant difference between the patients in initially-treated and smear positive group and in normal group,no difference was available among other groups.The factor(initially-treatment,retreatment)may have an effect on CD4+T cell count(P0.05).But the results between smear positive group and smear negative group had no difference(P0.05)in each test.Conclusion]CD4+T cell may play an important role in outcomes of tuberculosis patients.CD8+ T cell and humoral immunity play an important role in immune function of the tuberculosis patients.
