多弹头集束射频治疗肝癌的临床研究

目的　评价多弹头集束射频治疗肝癌的实用价值。方法　 7例原发性肝癌中 15个肝细胞癌结节(平均直径 3.5 2 cm) ,4例继发性肝癌中 7个转移癌结节 (平均直径 4.99cm)进行了 34次射频针电极插入、18次多弹头集束射频治疗。治疗后 7d复查腹部彩超 ,评价肿瘤是否完全坏死。结果　完成肿瘤结节射频治疗的平均次数 1.3,针电极插入的平均次数为 2 .3,每次射频治疗肿物热损伤的平均时间为 17.6 min。未观察到与治疗有关的并发症。治疗后 7d腹部彩超 ,2例肝转移癌 4个结节有肿瘤组织残留 ,进行了第 2次射频治疗。结论　多弹头集束射频治疗肝癌是一种安全有效的方法 ,直径不超过 4cm的肿瘤仅一次治疗就有效 ,大于 4cm短时间治疗有效而无并发症

Percutaneous treatment ofhepatic tumors by multiple radiofrequency needle electrodes

Objective To evaluate the usefulness of multiple RF needle electrodes in the treatment of hepatic cancer.Methods Eleven patients,7 of whom had 15 hepatocellular carcinoma nodules and 4 of whom had 7 hepatic metastatic nodules,underwent treatment by RF interstitial thermal ablation with multiple expandable needle electrodes.Twenty two tumor nodules were treated in 18 RF interstitial thermal ablation sessions with 34 needle electrode insertions.Sonography was performed 7 days after the RF interstitial thermal ablation treatment to evaluate the necrosis of the treated tumor.Results The average of RF ablations on tumor nodules was 1.3,average insertions of needle electrodes were 2.3;a mean course was 17.6 min for each RF heat damage to the tumor,no troubles seen due to the therapy.After a seven day therapy the color ultrasonography showed there remained tumorous tissues (residual) in four nodules of two cases of metastatic liver cancer,to whom second RF ablation therapy was given.Conclusion RF interstitial thermal ablation of hepatic tumor by expandable needle electrodes is a safe and effective method.Local ablation of tumors not exceeding 4.0 cm in diameter can be achieved only in one time without complications.
