腹腔镜治疗未生育妇女卵巢子宫内膜异位囊肿

目的 探讨腹腔镜治疗卵巢子宫内膜异位囊肿的可行性、技术要点及对妊娠的影响。方法 对151例患卵巢子宫内膜异位囊肿且尚未生育的患者，采用腹腔镜下卵巢子宫内膜异位囊肿剥离术，根据病情联合盆腔内异病灶电灼术、盆腔粘连松解术、输卵管通液术等治疗，随访术后复发率、受孕率、痛经缓解率等。结果 151例均在腹腔镜下顺利完成手术，无中转开腹，无任何手术并发症。平均手术时间45min，术中出血量平均50mL.术后随访6～36个月，痛经缓解总有效率为83．33％(40／48)．复发率为11．92％(18／151)，受孕率为24．03％(31／129)。结论 腹腔镜是诊断和治疗未生育妇女卵巢子宫内膜异位囊肿的最佳选择，特别是对伴有不孕症的患者治疗意义更大。术后联合药物治疗，可有效减少复发。

Evaluation of the treatment of ovarian endometriotic cyst by laparoscopic surgery in unbearing females

Objective To evaluate the feasibility and technique of laparoscopic surgery in treating the ovarian en-dometriotic cyst, and its influence to pregnancy. Methods One hundred fifty-one unbearing women with ovarian endometriotic cyst underwent oophorocystectomy with coagulation, adensionlysis, salpingoplasty or salpingostomy, utero-suspension by laparoscopy. The pain relief rate, recurrence rate, and pregnancy rate were followed. Results All 151 cases were successfully managed by laparoscopic surgery without complications. The average operation time was 45 min. The bleeding amount was about 50ml. We followed up these patients for 6-36 months. The pain relief rate was about 83. 33% , the recurrent rate about 11. 92% , and the pregnant rate 24. 03% . Conclusion Laparoscopy is the best way to diagnose and treat endometriosis in the unbearing women, especially the infertile patients. Medical treatment after operation can reduce recurrence of endometriosis efficiently.
