GPx-1基因Pro198Leu多态与2型糖尿病及合并冠心病的关系

摘    要:目的探讨谷胱苷肽过氧化物酶基因(GPx-1基因)Pro198Leu多态性与2型糖尿病(T2DM)及T2DM合并冠心病(CHD)的关系。方法以上海地区汉族人群中的198例T2DM患者和94名口服糖耐量试验(OGTT)正常的非DM对照者作为研究对象。其中T2DM组又分为无冠心病组(n=95)和冠心病组(n=103)。采用PCR扩增-直接测序法,分别检测各组GPx-1基因外显子2的Pro198Leu多态的基因型,并分析比较组间基因型、等位基因频率分布及临床变量间的差异。结果非DM对照组中,GPx-1基因Pro198Leu多态基因型以CC型、等位基因以C型为主,频率分布与文献报道中的瑞典人相比有显著差异(P=0.001);该多态基因型及等位基因频率分布,在无冠心病T2DM组与冠心病T2DM组间比较均无显著性差异(P>0.05);但与非DM对照组相比,T2DM组该多态CT基因型频率显著增加(P=0.012,OR=2.254)。结论上海地区汉族人群中,GPx-1基因Pro198Leu多态CT基因型个体发生T2DM的风险显著增加,但T等位基因并非T2DM发生冠心病的风险因素。

Abstract:Objective To explore the relationship between Pro198Leu polymorphism of glutathione peroxidase-1(GPx1) gene and type 2 diabetes mellitus(T2DM) and diabetic coronary heart disease (CHD). Methods Two hundred and ninety-two subjects in Han Chinese of Shanghai were divided into T2DM(n=198) group and OGTT normal non-DM controls(n=94).The T2DM group was subdivided into non-CHD(n=95) and CHD(n=103) group,respectively.The individual genotypes of Pro198Leu polymorphism in GPx-1 gene were determined by PCR-direct sequencing.Genotypic and allelic frequencies and clinical characteristics were compared among the groups. Results The most frequent genotype and allele of Pro198Leu polymorphism in GPX-1 gene in non-DM control group were CC and C in Chinese.Significant differences of genotypic and allelic frequencies in the polymorphism were observed between Chinese and Swedish populations(P=0.001).There were no significant differences in genotypic or allelic frequencies between CHD and non-CHD subgroups(P>0.05).Genotype CT frequency in T2DM group was significantly increased when compared with non-DM control group(P=0.012,OR=2.254).Conclusion Pro198Leu polymorphism of GPX-1 gene increases the risk of type 2 diabetes in Han Chinese of Shanghai,while T allele is not a risk factor of diabetic CHD.
