多导人工耳蜗植入术及听觉语言康复效果分析43例

目的总结43例多导人工耳蜗植入术围手术期的处理经验和听觉语言康复效果。方法介绍在多导人工耳蜗植入术围手术期的处理方法,分析比较语前聋与语后聋;学龄前聋儿与学龄聋儿人工耳蜗植入后的听觉语言康复效果。结果43例人工耳蜗植入术后通过1个月以上的调试和听觉训练,声场测听全部达到正常范围。语前聋儿童中,测定13例学龄前的可合作儿童的限定词表言语测听,言语识别率平均达到90%,非限定词表识别率平均达87%。测定9例7岁以上合作儿童的限定词表言语识别率平均达到82%,非限定词表言语识别率平均达78%,与学龄前组比较差异有显著意义。语后聋者经过一两个月的适应,全部能正常方式对话和打电话。结论人工耳蜗植入是治疗全聋患者的最佳选择。对语前聋患者而言,植入年龄最好在7岁前。人工耳蜗植入后,听觉语言康复工作将是一项长期的工作,需要家庭、社会、医生和聋康老师的共同努力。

Multichannel cochlear implants in 43cases and analysis on the effect of hearing and speech rehabilitation

Objective To summarize the treatment experien ce of multichannel cochlear implant ation and the effect of hearing and sp eech rehabilitation in 43cases.Methods To present the treatment methods of peri-operation of multichannel coch lear implantation,analyze and compare the effect of rehabilitatio n of hearing and speech between preli ngual and postlingual,preschool age and school age children.Results After mapping and auditory training in one month or more,the sound field h earing tests showed that their hearing level reached a normal range.The speech discrimination sc ore with close set vocabulary were 90%and with open set vocabulary were 87%in 13cases of preschool age children.The speech discrimination score with close set vocabulary were 82%and with open set were 78%in 9cases of school age children.There were significant di fference in statistics.The patient s of postlingual can dialogue and mak e a telephone call in normal way via adaptation of one to two months.Conclusion Cochlear implant is the best project for treat profound deafness.The age of operation should less than 7years for prelingual children.The hearing and speech rehabilitation after cochlear implantation is a long-term and hard work.It needs a great efforts made by family,society,surgeon and teacher of language re habilitation.
