急性ST段抬高心肌梗死患者溶栓治疗不同剂量氯吡格雷的临床效果比较

目的 分析不同剂量氯吡格雷治疗急性ST段抬高心肌梗死的疗效.方法 选取我院2009年3月至2011年3月收治的86例急性ST段抬高心肌梗死患者,随机分为2组.对照组43例,口服300mg氯吡格雷;研究组43例,口服600mg氯吡格雷;之后两组患者均按照75 mg/d持续口服氯吡格雷.观察比较两组患者的临床疗效.结果 治疗后研究组溶栓再通率为90.70％,明显高于对照组的74.42％,差异具有统计学意义（P＜0.05）;研究组肌酸激酶同工酶（CK-MB）峰值和达峰时间明显少于对照组,差异具有统计学意义（P＜0.05）.结论 起始剂量为600mg的氯吡格雷治疗急性ST段抬高心肌梗死的疗效要优于300 mg剂量的氯吡格雷,且具有较高的安全性.

Clinical effects comparison of thrombolysis with different doses of Clopidogrel in patients with acute ST segment elevation myocardial infarction

Objective To investigate the clinical effects of thrombolysis with different doses of Clopido- grel in patients with acute ST segment elevation myocardial infarction. Methods 86 patients with acute ST seg- ment elevation myocardial infarction were ehosen and randomly divided into two groups including control group（43 patients） with Clopidogrel for initial oral dose of 300 nag and study group （43 patients） with Clopidogrel for initial oral dose of 600 mg and then all patients have continuously oral administration of Clopidogrcl for 75 rag, and the clinical effects of both groups were compared. Results The reeanalization rate of study group （90.70%） was sig- nificant better than control group（74.42% ）（P〈0.05 ）. The peak levels and achieving time of study group was signif- icant lower than control group （P〈0.05）. Conclusion Compared with Clopidogrel for initial oral dose of 300 rag, Clopidogrel for initial oral dose of 600 mg in patients with acute ST segment elevation myocardial infarction possess better effects and higher safety.
