口腔颌面部动静脉畸形栓塞后应用环扎及电化学联合平阳霉素治疗的疗效评价

目的：评价环扎、电化学疗法联合平阳霉素注射治疗口腔颌面部软组织动静脉畸形的临床疗效。方法 ：2008年1月—2012年12月间采用环扎、电化学疗法联合平阳霉素注射治疗26例口腔颌面部软组织动静脉畸形,回顾分析其治疗效果及不良反应发生情况。根据疗效评价标准,将治疗结果分为治愈、好转、无效。结果：全组随访8个月-2 a,26例患者病变均不同程度缩小,有效率100%,临床治愈率77%（20/26）。术后不良反应主要是肿胀、疼痛、发热等,未见严重不良反应。结论：环扎、电化学疗法联合平阳霉素注射治疗口腔颌面部软组织动静脉畸形创伤小、无瘢痕,可重复治疗,外形和功能恢复良好,是一种有效的治疗方法。

Effect of arterial embolization followed by physical ligation,electrochemical therapy and pingyangmycin injection for arteriovenous malformations of oral and maxillofacial region

PURPOSE： To assess the clinical effects of physical ligation, eletrochemical therapy and pingyangmycin injection for arteriovenous malformations of oral and maxillofacial region. METHODS： Between January 2008 and December 2012, a total of 26 cases with arteriovenous malformations treated with electrochemical therapy, ligation and pingyangmycin injection were retrospectively analyzed. The follow-up time varied from 8 months to 24 months. The clinical outcome was divided into cure, improvement and no response according to the evaluation criterion of therapeutic effects. RESULTS： All the lesions decreased in size after the treatment, the clinical outcome of most patients（77%） was evaluated as cure. The most common complication was swelling followed by pain and fever, no serious adverse effects were encountered. CONCLUSIONS： Ligation, electrochemical therapy combined with pingyangmycin injection for oral andmaxillofacial arteriovenous malformations were reliable with less trauma, no scar, and could achieve satisfactory results with esthetically and functionally.
