睾丸精原细胞瘤伴血β-hCG增高一例报告并文献复习

目的 探讨伴血β-hCG增高的睾丸精原细胞瘤的临床及病理组织学特征.方法 应用光镜及免疫组化染色对1例伴血β-hCG增高的睾丸精原细胞瘤进行临床及病理组织学特征、诊断、鉴别诊断及预后分析,同时复习相关文献.结果 本例为典型精原细胞瘤,光镜下见弥漫的透明细胞,被纤维组织分隔,瘤细胞核仁明显,间质内见少量淋巴细胞,其间分布合体细胞;免疫组化CD117、D2-40、hCG、细胞角蛋白、合体滋养叶细胞阳性.结论 睾丸精原细胞瘤伴合体滋养叶细胞分化、血β-hCG增高是一种少见亚型,需引起临床及病理医生的重视,以避免误诊误治.

A Case of Seminoma of Testis with High β-hCG and the Literature Review

Objective To explore the clinicopathologic characteristics, diagnosis, differential diagnosis and prognosis of seminoma of testis with syncytiotrophoblastic giant ceils differentiation. Methods One case of seminoma of testis with syncytiotrophoblastic giant cells differentiation was analyzed with microscopy and immunohistochemsitry and with regard to cinicopathologic characteristics, diagnosis, differential diagnosis and prognosis. The literature was reviewed. Results This case was a typical seminoma, and characteristic microscopy feature was clear neoplastic cells, which was diffusely arranged and separated by fibrous tissue. Nucleoli was apparent, and a few lymphocytes were also seen. Among the neoplastic cells, syncytiotrophoblastis were seen and stained by CD117, D2-40, hCG and CK. Conclusion Seminoma of testis with Syncytiotrophoblastic giant ceils differentiation and high β-hCG is a rare subtype, and clinicians and pathologists should pay more attention to it and avoid misdiagnosis and mistreatment.
