影响听力重建手术疗效的相关因素分析

目的探讨听力重建手术疗效,并对其相关影响因素进行分析。方法对97例(99耳)慢性中耳炎患者均行开放式乳突根治并重建听骨链、成型鼓室,分析其临床资料,对可能影响听力重建疗效的10项因素进行logistic回归分析。结果纯音测听示术前气导平均听阈(47.3±9.0)dB,术后(32.1±8.7)dB;术前ABG平均为(31.7±9.3)dB,术后ABG平均为(16.7±8.8)dB。其中镫骨存在、咽鼓管通畅、鼓膜张肌腱存在对听力重建疗效具有统计学意义。结论多个因素影响听力重建手术的疗效,其中镫骨情况、鼓膜张肌腱、咽鼓管功能是较为主要的因素。

Factors Affecting Hearing Results Following Canal Wall-Down Tympanoplas

Objective To study factors affecting treatment outcomes after hearing reconstruction. Methods Ninety seven cases (99 ears) of chronic otitis media or cholesteatoma that underwent canal wall down mastoidectomy and hearing reconstruction were reviwed. Results The average air conduction pure tone threshold was 47.3±9.0 dB HL and 32.1±8.7 dB HL, and the air-bone gap was 31.7±9.3 dB and 16.7±8.8 dB before and after surgery, respectively. Multivariate analysis with logistic regression was used to analyze factors affecting outcomes. Existence of stapes and tensor tympanic muscle, normal eustanchian tube function of were favorable predictive factors. Conclusion Auditory results after tympanoplasty are influenced by multiple factors , Existence of stapes and tensor tympanic muscle, and normal eustanchian tube function favor good hearing results .
