冠状动脉粥样硬化钙化的电子束CT检测:一种新的钙化算法的探讨

摘    要:电子束CT(Electron beam computed tomography.EBCT)能定性定量地检测冠状动脉粥样硬化中的钙化,而钙化与冠状动脉粥样硬化程度正相关.并与冠心病事件的发生有一定的联系,因此,它是一种较好的无创性冠心病危险因子的检查方法。EBCT现行采用的钙化参数(钙化面积、钙化积分)可重复性较低,变异率较高,在使用中存在着一定的不足。本研究利用EBCT冠状动脉扫描图像上所有钙化像素点的三维信息,设计一种新的钙化参数(钙化体积),通过对11例人冠状动脉标本实验,发现钙化体积的变异率在25%百分位数、中位数、75%百分位数、均数处都较钙化面积、钙化积分小,其均数变异率差别的f检验尸值分别为0.027、0.058。结果显示钙化体积可作为一种新的钙化参数。

Abstract:Electron beam computed tomography(EBCT) can detect the atherosclerotic calcification of coronary artery qualitatively and quantitatively.It was also verified that the atherosclerotic calcification was directly related to the atherosclerotic extent and had a limited relation to the occurrence of coronary heart disease(CHD).So EBCT is one of the good non-invasive methods for predicting the risk of CHD.However,there are some problems in the calcification parameters(calcification area,calcification score) adopted by EBCT which have high variability and low reproducibility.As a result,these parameters have imperfection and need to be improved further.This research provides a new calcification parameter(calcification volume) which makes the use of three dimensional information of all cacific pixels in EBCT scanning images of coronary artery.After experiment in 11 human coronary artery specimens,it was testified that calcification volume had a lower variability than calcification area and calcification score in 25% percentile,median,75% percentile,Mean,respectively.P value of t test in Mean variability is 0.027,and 0.058.These results suggest that calcification volume may be a new calcification parameter.
