病灶清除植骨融合闭式冲洗联合内固定治疗胸腰椎结核

目的评价一期后路内固定,前路病灶清除植骨融合闭式冲洗引流治疗胸腰椎结核的临床疗效。方法采用一期后路内固定前路病灶清除植骨融合闭式引流治疗胸腰椎结核患者29例,按照FRANKEL分级评价患者手术前后的神经功能,根据X线片评价植骨融合时间,测量术前术后脊柱后凸角度及随访期内的角度丢失。结果29例患者切口均一期愈合,无窦道形成,结核治愈无复发,后凸角度平均纠正18.6度,X线显示所植骨骨性融合时间平均6～8个月。结论病灶清除植骨融合闭式冲洗联合内固定治疗胸腰椎结核的临床疗效确切,是治疗胸腰椎结核的有效方法。

Cleaning focus of infection,coalescing bone graft,closed type douching and internalfixation in treating tuberculosis of lumbar spine and thoracic vertebrae

Aim To appraise the clinical curative effect of internal fixation,cleaning focus of infection,coalescing bone graft and closed type douching in treating tuberculosis of lumbar spine and thoracic vertebrae.Methods Twenty-nine cases were treated by cleaning focus of infection,coalescing bone graft,closed type douching and internal fixation.Perioperative neural function was evaluated according to Frankel ranking,time of bone graft confluence was estimated by x-ray,perioperative ithyokyphosis angulation and loss of angulation during follow-up were measured.Results Twenty-nine cases all achieved primary intention healing.None of them had sinus tract and palindromia.Ithyokyphosis angulation was corrected to an average of 18.6°,and average time of bone graft confluence was from 6 months to 8 months.Conclusion Cleaning focus of infection,coalescing bone graft,closed type douching and internal fixation had certain clinical therapeutic effect in treating tuberculosis of lumbar spine and thoracic vertebrae.
