摘 要: | 目的探讨腹腔造影对盆底腹膜疝的诊断价值及临床应用。方法对23例盆底腹膜疝患者的腹腔造影结果和临床资料进行回顾性分析,其中女性21例,男性2例。测量盆底腹膜的位置,女性测量Douglas腔底部到阴道标志物的距离并将其分度。结果23例患者力排时盆底腹膜位置低于耻尾线,且与静坐相比,盆底腹膜位置下降程度均大于2.5cm。21女性患者Douglas腔底低于阴道后穹窿标志物顶点,按其低于程度将其分为3度,比阴道标志物上缘低0.5~1.5cm为轻度,有15例,中度1.5~3.0cm,有4例,重度超过3.0cm以上,有2例。结论腹腔造影能清楚显示盆底腹膜轮廓,对盆底腹膜疝有较大诊断价值。

Abstract: | Objective To explore the diagnostic value and clinical application of peritoneography in the diagnosis Pelvic floor peritonecele. Methods Imaging results of peritoneography and clinical date of 23 cases (female,n=21, male,n=2) with pelvic floor peritonecele were reviewed and analyzed retrospectively.The position of pelvic floor peritoneum of the whole examinee was measured, The distance of Douglas pouch to the marker within the vagina was measured and graded in female. Results The results showed that the position of pelvic floor peritoneum on the condition of maximum straining was below the pubococcygeal line on 23 cases , the decreasing degree of which aU exceeded 2.5cm compared with stillness state in sitting position. In 21 female patients, Douglas pouch was below the superior margin of the marker within vagina .According to the discending level of the inferior margin of Douglas cavity,mild (0.5-1.5cm,n=15),moderate (1.3-3.0), and servere (〉3.0cm,n=2) were classlfyed. Conclusion Profile of pelvic floor peritoneum can be clearly displayed by peritoneography,which play an important role in the diagnosis Pelvic floor peritonecele. |
