Clinical analysis of interlocking intramedullary nail in the treatment of double forearm fractures

Objective To analyze and compare the clinical efficacy of intramedullary nail and plate screw internal fixation in the treatment of forearm ulnar and radial shaft fractures.Methods The clinical data of patients with forearm ulnar and radial shaft fractures treated with interlocking intramedullary nail and plate from January 2013 to December 2017 were retrospectively analyzed.The general information(age,gender,fracture type),intraoperative conditions(bleeding volume,incision length,intraoperative C-arm fluoroscopy times),postoperative efficacy(complications,fracture healing time,function)were compared between the two groups.Results A total of 86 cases met the inclusion criteria,including 40 cases in the intramedullary nail group and 46 cases in the plate group.There was no significant difference in age and gender between the two groups(P>0.05).However,the intramedullary nail group had shorter preoperative waiting time,less blood loss,more fluoroscopy times,shorter hospitalization time,and less proportion of simple fractures(type A3),compared with the plate group,and the differences were statistically significant(P<0.05).There was no significant difference in average fracture healing time,DASH score,Grace-Eversman score between the intramedullary nail group and the plate group(P>0.05).Although the incidence of postoperative complications was low and the satisfaction rate was relatively high in the intramedullary nail group,there was no significant difference between the two groups(P>0.05).Conclusion Compared with plate and screw,intramedullary nail has the advantages of shorter waiting time,less blood loss,shorter incision length,fewer postoperative complications and higher patient satisfaction rate in the treatment of forearm ulnar and radial fractures.However,a multicenter,randomized controlled study is needed to further confirm the efficacy comparison between the two groups.
