婴幼儿左向右大分流先天性心脏病的外科治疗

目的 :评估婴幼儿左向右大分流先心病外科手术治疗的效果。方法 :回顾分析 1999- 0 1～ 2 0 0 3- 0 1手术治疗婴幼儿左向右大分流先心病 12 8例。均一期手术 ,病种为室间隔缺损、房间隔缺损、动脉导管未闭、单心房等。结果 :术前的营养支持 ,术中的精细操作 ,术后的正性肌力药物及强效肺血管扩张剂 PGE1 的合理使用 ,持续静注速尿 ,维持水电解质平衡是手术成功的关键因素。随访 1个月～ 3年 ,全组病例均获得了较好效果。结论 :左向右大分流先心病伴有肺炎、心衰及并发肺动脉高压 ,经内科治疗无效时应及时手术治疗。

Repair of large left-to-right shunting the congenital heart disease in infants

AIM:To evaluate the efficacy of large left-to-right shunting repair in infants. METHODS: From January 1999 to January 2003,128 cases received one-staged operation. In this group, ventricular septal defects, atrial septal defects, patent ductus arteriosus and single auricle were involved. RESULTS: The preoperative nutritional, deliberate operative skill, postoperative using of inotropic drug and PGE-1 are the key factors to a successful operation. All patients were followed-up from 2 months to 3 years with satisfactory results. CONCLUSION: The congenital heart disease left-to-right usually accompanys pneumonia, heart failure and pulmonary hypertension. Surgical repair should be taken at once if physical treatment fails.
