128层CT头颈部DSA:不同管电压及对比剂浓度的对照研究

摘    要:目的:对比不同管电压及对比剂浓度在头颈部MSCT数字减影血管造影(MSCTDSA)检查中的可行性。方法:42例行MSCTDSA的患者平均分为A、B组,采用不同电压和不同浓度对比剂行平扫和增强扫描并记录数据,采用4分法对颅内动脉分支显示情况进行评价并行统计学分析。结果:2组相比,平扫和增强扫描的CT容积剂量指数差异均有统计学意义(P0.05),射线剂量分别降低约53.7%、33.0%,上矢状窦强化程度差异有统计学意义(P0.05),其余所测2组各目标血管CT值差异无统计学意义。大脑中动脉分支显示情况评分差异无统计学意义(P0.05)。结论:头颈部MSCTDSA技术采用低电压及低浓度对比剂扫描方案有重要的临床应用价值。

Abstract:Objective:To compare the feasibility of multi-slice spiral CT digital subtraction angiography (MSCTDSA) with different low-kilovoltage and concentration contrast media in the. head and neck. Methods: Forty-two patients performed by MSCTDSA scan in the head and neck were randomly and averagely divided into group A (100 kV, 370 mgI/mL contrast 45 mL) and group B (80 kV,300 mgI/mL contrast 45 mL). The radiation dose,intravascular CT attenuation values of carotid and in- tracranial vessels were recorded. Visualization of intracranial artery was rated on four-point grading scale. Statistical difference was determined by using t-test with the SPSS 13.0 software. P〈0.05 was considered statistically significant. Results: Comparing the two groups,the CTDIvol in plain and contrast scan had significant difference (P〈0.05). The radiation dose decreased 53.7% and 33.0% respectively. The intravascular CT attenuation values of superior sagittal sinus had significant difference (P〈0.05) ,while the ones of the other target vessels had not. The scores of the visualizations of intracranial artery were not statistically significant different between the two groups (P〉0.05). Conclusion:MSCTDSA with low-kilovoltage and low concentration contrast media in the head and neck can maintain the image quality, reduce radiation dose and contrast lesions, and lessen the CT attenuation values of intracranial veins,which have great potential in clinical application.
