D型人格冠心病患者应付方式、负性情绪的研究

目的 探讨冠心病患者中D型人格者的分布及其应付方式、负性情绪的特点.方法 收集203例冠心病患者为病例组,在普通健康人群选取199例为对照组,采用D型人格量表(DS14)、应付方式问卷、汉密尔顿抑郁量表、汉密尔顿焦虑量表进行评估,并进行统计分析.结果 (1)冠心病组的D型人格者比例(38.9%,25.5%,x2=8.192,P＜0.01)、DS14总分[(21.17±9.52),(19.31±7.30),P＜0.05]、负性情感(NA)评分[(10.35±5.97),(2.32±0.02),P＜0.05]高于对照组,社交抑制(SI)维度评分冠心病组和对照组差异无显著性[(10.82±4.99),(10.21±3.94),P＞0.05].(2)D型人格者在应付方式问卷中解决问题维度评分低于非D型人格者(P＜0.01),自责、幻想、退避、合理化维度评分高于非D型人格者(P＜0.01).(3)冠心病组中D型人格者合并抑郁、焦虑的比例和评分高于非D型人格者.结论 D型人格者较多运用消极的应付方式,较少运用积极的应付方式,更多体验焦虑和抑郁等负性情绪.

Study on the coping style and negative mood of type D persons in the patients with coronary heart disease

Objective To discuss the distribution of type D persons and their characteristics of coping style,negative mood in the patient with coronary heart disease(CHD).Methods 203 patients with CHD,and 196 healthy controls selected from the general population were investigated by the type D Scale-14(DS14),coping style questionnaire,HAMD,HAMA.Results The proportion of type D persons in the CHD patients Was higher than controls(38.9%vs 25.5%,x2=8.192,P＜0.01).CHD patients scored higher on DSl4 and NA than their counterparts in control group(10.35±5.97 vs 2.32±0.02,P＜0.05).There was no significant different between the SI scores of CHD patients and controls(10.82±4.99 vs 10.21±3.94,P＞0.05).Type D persons scored lower on problem-resolving(尸＜0.01),higher on fantasy,blench,rationalization,self-abuse(P＜0.01).Type D persons in the CHD group scored higher on HAMD(P＜0.01)and HAMA(P＜0.01)than the non-type D persons.Conclusion Typo D persons use the positive coping style less than non-type D persons,and use the negative coping style more than non-type D persons.Type D persons experienced depression and anxiety more.
