个案管理对重性精神病患者健康状况和生活质量的影响

目的：探讨个案管理综合干预模式对重性精神病患者健康状况和生活质量的影响.方法：将80例重性精神病患者,随机分为个案管理组和基础治疗组,每组各40例.个案管理组结合患者情况制定个性化方案,予以综合干预措施;对照组仅维持基本的药物治疗.使用生活质量综合评定问卷（GQOLI-74）、社区重性精神病患者安全风险评估量表（CSPPRAS）、临床疗效总评量表（CGI-SI）进行评估.结果：1年末随访时个案组与基础组比较,GQOLI总分、躯体功能与心理功能维度评分差异较为显著;CSPPRAS总分及肇事肇祸史、近1个月行为异常表现、情绪情感表现评分差异较为显著;CGI-SI量表疗效总评差异较为显著.结论：个案管理综合干预模式能有效改善重性精神病患者的病情及生活质量,降低肇事肇祸率,促进患者社会功能的恢复.

Effects of case management on health status and quality of life for patients with major psychosis

Objective：To explore effects of comprehensive intervention mode of case management on health status and quality of life for patients with major psychosis. Methods：80 cases with major psychosis were randomly divided into case management group （40 cases） and basic treatment group （40 cases）. Case management group were given a personalized treatment and comprehensive in- tervention measures, whereas control group only took the basic medication. General quality of life inventory （ GQOLI-74）, severe psy- chiatric patients in community safety risk assessment scale （ CSPPRAS）, clinical global impression scale （CGI-SI） assessment were used to assess the curative effects. Results： I year later, the follow-up study found that： the difference of the GQOLI total score and physical function and psychological function dimension score between the two groups were significant; the difference of the CSPPRAS total score, troublemaking history, and abnormal behavior and emotional performance score within 1 month between the two groups were significant; and the difference of CGI-SI scores between the two groups was significant. Conclusions： For the patients with major psy- chosis, the comprehensive intervention mode of case management can effectively improve the patients＂ conditions and quality of life, re- duce the accident rate, and promote the recovery of social function.
