急性一氧化碳中毒后迟发性脑病患者事件相关电位检测的临床价值

摘    要:目的探讨急性一氧化碳中毒后迟发性脑病(DEACMP)患者的认知功能状况,为其临床寻找客观的实验室依据。方法将66例急性一氧化碳中毒患者根据病情进展是否发生DEACMP划分为DEACMP组和非DEACMP组。分别对2组患者进行了事件相关电位(ERP)检测和长谷川简易智能量表(HDS)评分,并将结果进行比较。结果与非DEACMP组比较,DEACMP组ERP测定中N2、P3波潜伏期延长,P3波幅降低,差异有统计学意义(P<0.01);DEACMP组HDS评分较非DEACMP组明显降低,差异有统计学意义(P<0.01)。DEACMP组ERP成分中P3波潜伏期与HDS评分呈负相关(r=-0.45,P<0.01),而P3波幅与其呈正相关(r=0.44,P<0.01)。结论 DEACMP患者存在明显的认知功能障碍,ERP可作为评价DEACMP患者认知功能的实验室指标应用于临床。

Clinical value of event related potential test for delayed encephalopathy after acute carbon monoxide poisoning patients
Abstract:Objective To explore the cognition of patients with delayed encephalopathy after acute carbon monoxide poisoning(DEACMP),in order to find the laboratory indexes for the clinic.Methods A total of 66 patients with acute carbon monoxide poisoning were divided into DEACMP group and non-DEACMP group according to whether making advancement of delayed encephalopathy or not.Event-related potential(ERP)and Hasegawa dementia scale(HDS) were used for the two groups and the results were compared between them.Results Compared with the non-DEACMP group,the latency of N2 and P3 of ERP were significantly longer and the amplitude of P3 was significantly lower in the DEACMP group(P<0.01).The scores of HDS in the DEACMP group were significantly lower than those in the non-DEACMP group(P<0.01).Correlation analysis showed that there was a remarkable negative correlation with the scores of HDS and latency of P3 of ERP in the DEACMP group(r=-0.45,P<0.01),and a significant positive correlation with the amplitude of P3(r=0.44,P<0.01) respectively.Conclusion DEACMP patients have the cognitive dysfunction obviously.ERP test can be used as clinical indexes of cognition for patients with DEACMP.
