紫杉醇或顺铂联合氟尿嘧啶同步三维适形放射治疗中晚期食管癌疗效比较

目的评价紫杉醇或顺铂联合氟尿嘧啶同步三维适形放射治疗中晚期食管癌的疗效和毒副反应。方法采用同期对照方法,共96例中晚期食管癌患者入组,其中46例入组紫杉醇联合氟尿嘧啶同步三维适形放射治疗组(观察组),50例入组顺铂联合氟尿嘧啶同步三维适形放射治疗组(对照组),比较2组的近期疗效、远期疗效及毒副反应。结果近期疗效比较,观察组疾病控制率与对照组比较差异无统计学意义(P>0.05),客观缓解率高于对照组(P<0.05)。远期疗效比较,观察组的中位生存期、无进展生存期均高于对照组(P<0.05)。观察组消化道反应低于对照组,但观察组在Ⅲ～Ⅳ度食管炎及乏力发生率方面高于对照组,差异均有统计学意义(P<0.05),观察组和对照组脱发、嗜睡、便秘、水肿等毒副反应发生率差异无统计学意义(P>0.05)。结论紫杉醇联合氟尿嘧啶同步三维适形放射治疗较顺铂联合氟尿嘧啶同步三维适形放射治疗客观缓解率增高,能够延长生存时间并改善预后,且毒副反应无明显增加。

Paclitaxel or cisplatin combined with fluorouracil concurrent three-dimensional radiotherapy for treatment of advanced esophageal cancer

Objective To evaluate the therapeutic effects and toxicities of paclitaxel or cisplatin combined with fluorouracil concurrent three-dimensional radiotherapy for treatment of advanced esophageal cancer.Methods This study was carried out as a concurrent control clinical trial.A total of 96 patients with advanced esophageal cancer were included this study.Forty-six patients were given paclitaxel combined with fluorouracil concurrent three-dimensional radiotherapy as observation group;fifty patients were given cisplatin combined with fluorouracil concurrent three-dimensional radiotherapy as control group.The short-term effects,long-term effects and toxicities were observed in the two groups.Results The short-term results were that the disease control rates between the two groups were not significant difference(P>0.05),but the objective remission rates in observation group were higher than those in the control group(P<0.05).The long-term results showed that the median overall survival and the progression free survival time of observation group were higher than those in the control group(P<0.05).The digestive tract reaction in the observation group was significantly lower than that in the control group,but the occurence rates of Ⅲ-Ⅳ oesophagitis and asthenia were significantly higher than those in the control group(P<0.05).The incidence of hair loss,lethargy,constipation,and edema between the observation group and control group had no statistically significant(P>0.05).Conclusion Paclitaxel combined with fluorouracil concurrent three-dimensional radiotherapy for locally advanced esophageal has higher objective remission rate than cisplatin combined with fluorouracil concurrent three-dimensional radiotherapy,it can prolong survival time significantly and improve prognosis,and the toxicities don′t increase.
