急性缺血性脑卒中的磁敏感加权成像研究

目的探讨磁敏感加权成像（SWI）在急性缺血性脑卒中（AIS）合并脑微出血（CMBs）的价值。方法 80例AIS患者行常规MRI及SWI检查后进行相关对照研究。结果 （1）80例AIS发生CMBs的28例,发生率35%;出现出血性转化（HT）10例,HT合并CMBs 6例;病变内及周围静脉增多/减少。（2）AIS病灶内及周围静脉血管变化及HT与CMBs没有明显相关性;CMBs数目在AIS同侧或对侧区域的分布没有显著性差异（P〉0.05）。（3）SWI图CMBs呈≤10 mm的圆形、卵圆形的低信号或以低信号为主的混杂信号。结论 SWI在AIS合并CMBS的诊断中具有重要价值,应该成为AIS合并CMBs患者的首选影像学检查方法。

Application of susceptibility weighted imageing in diagnosis acute ischemic stroke

Objective： To investigate the value of susceptibility weighted imaging（SWI） in diagnosis of acute ischemic stroke（AIS） with cerebral microbleeds（CMBs）.Methods： Eighty patients with acute ischemic underwent conventional MRI and SWI sequence,to compare and study the relation between acute ischemic and cerebral microbleeds.Results ：（1） 28 CMBs occurred in 80 AIS patients,35% of CMBs in AIS;AIS with hemorrhagic transformation（HT） in 10 patients,HT with CMBs in 6 patients;The veins in and periphery AIS increased or decreased.（2） There was no releation between the veins and HT with CMBs;The number of CMBs does not have significant difference with the same side or the opposite side distribution of the AIS region（the P0.05）.（3） CMBs showed rounded or ovaled foci of ≤10mm in size that appeared hypointensity or mixted-single intensity in SWI data.Conclusion ：SWI has an important value in acute ischemic stroke with CMBs,and SWI sequence should be the first choice in the diagnose of stroke with CMBs.
