后腹腔镜肾部分切除术和开放肾部分切除术治疗肾肿瘤的疗效比较

目的比较后腹腔镜肾部分切除术(LPN)和开放肾部分切除术(OPN)治疗肾肿瘤的疗效。方法选择2011年5月至2013年8月间接受手术治疗的肾肿瘤患者68例,将患者采用随机数字表法分为观察组和对照组,每组34例。其中观察组患者接受LPN治疗,而对照组患者接受OPN治疗,比较两组患者术中情况和术后恢复情况。结果观察组患者手术时间、术中失血量和引流管拔除时间分别为(132.4±42.3)min、(224.5±109.2)ml和(5.4±1.6)d,对照组患者分别为(136.3±41.7)min、(236.1±101.4)ml和(5.8±1.3)d,组间差异无统计学意义(P>0.05)。观察组患者术后住院时间、肠道恢复时间和术后1周视觉模拟评分(VAS)均明显低于对照组,差异有统计学意义(P<0.05)。两组患者并发症发生率比较差异无统计学意义(P>0.05)。术后随访1年,两组患者均未出现局部复发。结论 LPN治疗肾肿瘤具有术后恢复快的优势,术后疼痛更轻,是科学且安全可靠的手术方式。

Comparative analysis of laparoscopic partial nephrectomy and open partial nephrectomy for patients with renal tumors

Objective To analyse the efficacy of laparoscopic partial nephrectomy （LPN） and open partial nephrectomy （OPN） for patients with renal tumors. Methods From May 2011 to Augest 2013, sixty-eight patients with renal tumors accepting surgical treatment in Hunan Provincial People＇s Hospital were selected and were randomly divided into the observation group and the control group, 34 cases in each group. The patients in the observation group accepted LPN, while the patients in the control group received OPN. Then intraoperative situations and postoperative recovery of two groups were recorded and compared. Results Operative time, intraoperative blood loss and drainage tube removal time was （132.4 ± 42. 3 ） rain, （224. 5± 109. 2） ml, （5.4 ± 1.6） d in the observation group, and were （ 136. 3 ± 41.7 ） min, （236. 1 ± 101.4）ml, （5.8 ± 1.3）d in the control group, and the difference was not statistically significant （P 〉0. 05 ） ; Postoperative hospital stay, intestinal recovery time and visual analogue scale （VAS） score in 1 wk postoperative in the observation group were significantly lower than those of the control group （P 〈 0. 05 ） ; There was no significant difference between the two groups in incidence rate of complication （ P 〉 0. 05 ）, and there was no local tumor recurrence after 1 year in the two groups. Conclusion LPN method has the advantage of quick recovery, and lighter pain after operation, and it is a safe and reliable surgical approach.
