三维适形放射治疗联合多西紫杉醇和顺铂治疗晚期非小细胞肺癌的疗效分析

目的探讨三维适形放射治疗联合多西紫杉醇和顺铂(TP方案)化疗治疗晚期非小细胞肺癌(NSCLC)的临床疗效和安全性。方法选取2010年3月至2013年3月间收治的60例晚期NSCLC患者,随机分为对照组和观察组,每组30例。对照组患者给予单纯TP方案化疗,观察组患者给予三维适形放射治疗联合TP方案化疗,观察两组患者的临床疗效、1年、2年生存率和不良反应发生情况。结果对照组患者治疗总有效率为56.7%,观察组患者为83.3%。对照组患者的1年和2年生存率分别为53.3%和40.0%,生存时间为(10.9±1.7)个月;观察组患者的1年和2年生存率分别为80.0%和56.7%,生存时间为(18.6±2.1)个月。两组患者早期放射性反应主要为Ⅰ^Ⅱ级放射性食管炎和骨髓抑制,晚期放射性反应主要为ⅠⅡ级放射性食管炎和骨髓抑制,晚期放射性反应主要为Ⅰ^Ⅱ级放射性肺炎。结论三维适形放射治疗联合TP方案化疗治疗晚期NSCLC,可有效提高患者的近期疗效,延长患者的生存时间,不良反应可以耐受,但有增加的趋势。

The analysis of treatment efficacy and safety conformal radiotherapy in combination with TP chemotherapy in advanced non-small cell lung cancer

Objective To explore the clinical efficacy and safety of three dimensional conformal radiotherapy combination with TP chemotherapy treatment of advanced non-small cell lung cancer. Methods Selected 60 cases of advanced non-small cell lung cancer patients in our hospital from 2010 March to 2013 March, 60 cases were randomly divided into control group and observation group, control group gave TP chemotherapy alone, the observation group gave the three dimensional conformal radiotherapy in combination with TP chemotherapy, observed two groups of clinical curative effect, 1 year and 2 year survival rate, and adverse reactions occur. Results The total effective rate of control group was 56. 7%, observation group was 83.3% , 1 year and 2 year survival rates of control group were respectively 53.3% and 40. 0% , median survival period was （ 10. 9 ± 1.7 ） months, 1 year and 2 year survival rates of observation group were respectively 80% and 56. 7% median survival period was （ 18.6 ±2. 1 ） months, two groups of early radioactive reaction was mainly Ⅰ -Ⅱ level radioactive esophagitis and bone marrow suppression, advanced level radioactive reaction was mainly Ⅰ -Ⅱ radioactive pneumonia. Conclusion Three dimensional conformal radiotherapy combinations with TP chemotherapy to treat advanced non-small cell lung cancer, which can effectively improve the patient＇s recent curative effect, extend the patients survival, tolerate adverse reactions, but have a tendency to increase.
