摘    要:目的研究急性心肌梗死(AMI)患者血浆脑钠肽(BNP)水平与梗死部位和心功能的关系。方法选择2012年6月—2013年6月在我科住院的ST段抬高AMI患者50例为AMI组,同期在我院体检健康者50例为对照组。检测两组受试者的血浆BNP水平,分析其与梗死部位、左室射血分数(LVEF)及Killip分级的关系。结果 AMI组患者血浆BNP水平为(6 357.07±3 772.57)ng/L,明显高于对照组的(65.75±64.36)ng/L(P0.05)。不同梗死部位AMI患者血浆BNP水平比较,差异无统计学意义(P0.05)。单因素COX回归分析结果显示,血浆BNP水平与LVEF呈负相关(OR=-0.47,P0.05)。不同Killip分级AMI患者血浆BNP水平比较,差异有统计学意义(P0.05)。结论 AMI患者血浆BNP水平明显升高,其与梗死部位无关,与LVEF及Killip分级相关,可作为评价AMI患者心功能的参考指标。

Relationship of Plasma Brain Natriuretic Peptide Level to Infarction Positions and Heart Function in patients with Acute Myocardial Infarction

Abstract:Objective To study the relationship of plasma brain natriuretic peptide(BNP) level to infarction posi-tions and heart function in patients with acute myocardial infarction(AMI). Methods From June 2012 to June 2013,50 AMI patients with ST - segment elevation in our hospital were selected as AMI group,and 50 healthy people selected as control group. Plasma BNP level was detected in AMI group and control group,and its correlation with infarction positions,left ventricu-lar ejection fraction( LVEF) and Killip systematics was analyzed. Results The plasma BNP level in AMI group was(6 357. 07 ± 3 772. 57)ng/ L,was significantly higher than that in control group of(65. 75 ± 64. 36)ng/ L(P 〈 0. 05). There was no significant differences of plasma BNP levels in AMI patients with different infarction positions(P 〉 0. 05),but there was in AMI patients with different Killip systematics(P 〈 0. 05). By univariate COX regression analysis,plasma BNP level was neg-atively correlated with LVEF(OR = - 0. 47,P 〈 0. 05). Conclusion Plasma BNP level is significantly elevated in AMI pa-tients,and is not correlated with infarction positions,but with LVEF and Killip systematics,and it can be used as an important predictor for left ventricular function in AMI patients.
