临终关怀护理对癌症晚期患者临终期生存质量的影响

目的探讨临终关怀护理对癌症晚期患者临终期生存质量的影响,为临终关怀护理在临床上进一步普及提供依据。方法选取2011年1月至2013年12月收治住院的临终癌症晚期患者73例。按照患者及家属意愿将73例患者分为对照组(选择继续治疗,24例)和观察组(放弃治疗,进行临终关怀,49例)。采用生存质量核心问卷(QLQ-C30)量表评价患者的生存质量,并调查患者对护理工作的满意度。对照组实施常规护理,观察组实施临终关怀护理。比较两组患者的生活质量及对护理人员的满意率。结果两组患者临终关怀前生活质量评价比较,差异无统计学意义(P>0.05)。临终关怀1个月后,观察组患者的躯体功能、情绪功能、认知功能及整体生活质量评分均明显优于对照组(均P<0.05);观察组患者对护理工作满意度为71.4%,明显高于对照组(33.3%,P>0.05)。结论临终关怀护理能提高癌症晚期患者临终期生存质量,值得临床推广。

Impact of hospice care for quality of life in patients with advanced cancer

Objective To investigate the impact of hospice care for quality of Life in patients with advanced cancer.Methods 73 patients with advanced cancer were admitted to this study in affiliated hospital of Nanjing medical university from January 2011 to December 2013.In accordance with the wishes of patients and their families,73 patients were divided into a control group（ choose to continue treatment） 24 cases and observation group（ to give up treatment,carried hospice） 49 cases.The quality of life questionnaire lung cancer 13（ QLQ-C30） scale used to evaluate the quality of life of patients and survey patient satisfaction of nursing work.Results The difference of quality of life scale in two groups of patients before hospice care was not statistically significant（ P〈 0.05）.Physical function,emotional function,cognitive function and overall quality of life in observation group after hospice month were significantly better than the control group（ t = 11.358,9.537,8.226,10.135,P〈 0.05）.The patients of nursing job satisfaction was 71.4% in observation group,significantly higher than the 33.3% in control group（ X2= 37.430,P〉 0.05）.Conclusion Hospice care can improve the survival quality of patients with advanced cancer,worthy of promotion.
