循证方法在老年骨科手术患者术前1日饮食护理中的应用

摘    要:目的:探讨和评价循证法与常规法在老年骨科手术患者术前1d饮食护理中的效果。方法:选择老年骨科手术患者85例,根据循证的结果将实施术前1d饮食护理的40例患者作为实验组,实施常规护理的45例患者作为对照组,对照组采取常规方法实施术前饮食护理,患者在术前晚10:00开始禁食禁饮;实验组根据循证结果,患者在术前8h禁食,4h禁饮。结果:实验组患者饥饿程度、口渴度降低,舒适度、满意度提高(均P〈0.05);对照组组内术前血糖与基础血糖比较差异有统计学意义(均P〈0.05),实验组组内术前血糖与基础血糖比较差异无统计学意义;实验组及对照组患者均未出现因患者术前禁食禁饮时间过短而引起的手术改期及胃内容物反流。结论:根据循证结果对老年骨科患者实施术前日饮食护理,能提高手术患者的舒适度、满意度和手术的安全性且切实可行。

Abstract:Objective: To investigate and evaluate evidence-based method and conventional method in elderly patients undergoing orthopedic surgery the day before the effect of dietary care. Methods: 85 cases of elderly patients with orthopedic surgery, according to the results of the implementation of evidence-based care and diet the day before surgery, 40 cases of the test group, the implementation of routine care control group of 45 cases, conventional methods of implementation of the conventional group to take care of preoperative diet , the night before surgery in 10 patients began fasting drink; experimental group according to evidence-based results, patients fasted for 8 hours before surgery, 4 hours forbidden to drink. Results: The patients’ level of hunger, thirst was reduced, comfort , satisfaction increased (all P〈0.05); the control group and group-based preoperative blood glucose was statistically different (all P〈0.05), the experimental group and the group basis of preoperative blood glucose showed no significant difference; conventional group and normal group of patients did not appear due to preoperative fasting time was too short to drink due to surgery postponed, and regurgitation of gastric contents. Conclusion: Evidence-based implementation of the results of surgery in elderly orthopedic patients the day before diet care, surgery can improve patient comfort, satisfaction and safety of operation and practical.
