26例强直性脊柱炎患者髋关节受累的临床特征分析

目的：探讨强直性脊柱炎（Ankylosing Spondylitis,AS）患者发生髋关节病变的危险因素。方法：对26例有髋关节病变的AS患者（A组）和37例无髋关节病变的AS患者（B组）的临床特征进行对比。结果：A组的发病年龄早于B组（P〈0.05）。两组中以外周关节起病者,分别为73.1%（19/26）和35.1%（13/37）;ESR,CRP和IgG水平A组均高于B组（P〈0.05）。结论：发病年龄早,以外周关节起病,炎性指标高者易累及髋关节。在临床诊疗中,对存在上述疾病特点的患者,应及早进行髋关节的CT和MRI检查,以期尽早发现病变,积极治疗,减少致残。

Clinic Analysis of 26 Cases with Ankylosing Spondylitis Developing Hip Joint Disease

Objective: To analyze the possible risk factors for ankylosing spondylitis(AS) patients developing hip joint disease.Methods: 26 patients with hip joint disease(group A) were contrasted with 37 patients without hip joint disease(group B) in the clinic features.Results: The mean age at the time of disease onset in group A was younger than that in group B(P<0.05).Involvement of peripheral joints as the first signs in two groups were 73.1%(19/26),35.1%(13/37),respectively.ESR,CRP and IgG levels were higher in group A than those in group B(P<0.01).Conclusion: Patients with younger age at onset or with peripheral joint involvement are more likely to suffer from hip joint disease,and patients with involvement of hip joint at early disease course with higher levels of ESR,CRP and IgG,are predisposed to having destructive hip joint disease.Patients with these signs must inspect CT and MRI of hip joint in order to find earlier so as to reduce disability.
