气管隆突重建术的实验研究

研究了一种新的气管、隆突重建术,进行了实验研究。手术要点是仅开右胸,切除右肺中下叶,保留右上叶,隆突下切断左右主支气管,用右侧中间支气管近端与左主支气管远端吻合,右主支气管上提与气管近侧断端吻合。结果5只犬除1只因术中麻醉管脱出而致心脏骤停死亡外,余均存活,术后观察3个月无呼吸功能障碍,饮食、活动同正常犬。

THE EXPERIMENTAL STUDY OF TRACHEAL AND CARINAL RECONSTRUCTION

A new method of tracheal and carinal reconstruction was tested in 1985,the procedure was performed in 5 adult dogs.Throughright thoracotomy the intrathoracic trachea,carina and proximal parts ofthe left and right main bronchi were mobilized.A right middle-and-lowerlobectomy was performed in the standard fashion and the upper lobe pre-served.The lower part of the trachea with carnia was then removed.Theproximal end of the right truncus intermedius is anastomosed to the distalend of the resected left main bronchus,and the distal end of the resectedright main bronchus anastomosed to the proximal end of the resectedtrachea,the bifurcation of right main bronchus and right truncus inter-medius has naturally become the new carina at this time.Four animalssurvied the procedure without apparent functional difficulty and were ingood condition during the observation of postoperative three months.Webelieve that this method described can be used for the patients whosetracheal lesions have infiltrated the carina,and extend the resected limitsalthough some further studies yet remain to be done.
