脑出血患者血清VIP,CGRP,sICAM-1和sVCAM-1水平测定

摘    要:目的:探讨脑出血患者血清中血管活性肠肽(VIP)、降钙素基因相关肽(CGRP)、可溶性细胞间黏附分子-1(sICAM-1)和可溶性血管细胞黏附分子-1(sVCAM-1)等相关标志物水平的变化及临床意义。方法:采用放射免疫法测定VIP、CGRP含量,采用双抗夹心酶联免疫吸附法(ELISA)测定血清中sICAM-1、sVCAM-1含量。结果:脑出血患者第1天VIP、sICAM-1和sVCAM-1水平与对照组比较无显著改变(P〉0.05),CGRP含量显著高于对照组(P〈0.05);而在7d后水平与对照组及第1天比较除VIP有显著降低,其余3指标显著升高(均P〈0.05)。血清VIP在第1天除重型病例较对照组明显下降(P〈0.05),其他两型无明显变化(P〉0.05),第7天3组均有不同程度降低(P〈0.05)且病情越重增高越显著(P〈0.05);血清CGRP在第1天较对照组均明显升高并中重型病例第7天仍高于对照组(P〈0.05);血清sICAM-1、sVCAM-1水平在第1天均不增高(P〉0.05)但在第7天增高显著(P〈0.05)且病情越重增高越显著(P〈0.05)。结论:脑出血患者的血肿周围脑组织存在炎症反应,其测定可判断患者病情的严重程度和预后。

Abstract:Objective:To study the changes of serum levels of VIP, CGRP, sICAM-1 and sVCAM-1 in patients with intracerebral hemorrhage and its clinical significance. Method: Seventy patients with intracerebral hemorrhage were divided into 3 groups (mild group, moderate group and severe group). 60 healthy people were selected as normal control group. Their serum levels of VIP and CGRP were determined by radioimmunoassay. The serum levels of slCAM-1 and sVCAM-1 were determined by enzyme immunoassay. Result: At the first day, the serum levels of VIP,slCAM-1 and sVCAM-1 were not significantly higher in patients with intracerebral hemorrhage than those in the control group (all, P〉0.05). The serum level of CGRP were significantly higher in patients with intracerebral hemdrrhage than those in the control group (P〈0.05). The serum level of VIP decreased significantly after 7 days compared to the control group (P〈0.05) ,whereas the serum levels of CGRP,slCAM-1 and sVCAM-1 increased significantly compared to the control group(P〈0.05). At 1st day, the serum CGRP level was significantly higher in mild group than those in the control group (P〈0.05). The serum levels of VIP,sI- CAM-1 and sVCAM-1 were shown to be not much significantly higher than patients in mild group (all P〉0.05). In moderate group, the-changes of the 4 serum indexes at the Ist day were similar to patients in Mild group. The serum levels of the 4 serum indexes were significantly higher at 7th day than those in the control group (all P(0. 05). In severe patients group, the serum levels of VIP,CGRP at 1st day were significantly higher than those in the control groups (all P〈0. 05), but the serum levels of slCAM-1 and sVCAM-1 were not much difference than those in the control groups (all P):〉0.05). The changes of serum levels of the 4 serum indexes were similar to those in mbderate group after 7 day. Conclusion:Determination the changes of serum levels of VIP,CGRP,slCAM- 1 and sVCAM-1 in patients with intracerebral hemorrhage would contribute to evaluate the patient's condition and judge the prognosis.
