右心室射血分数与肺切除术后并发症的相关性分析

目的:探讨术前右心室射血分数(right ventricular ejection fraction,RVEF)与肺切除术后并发症的关系。方法:选取上海交通大学胸科医院胸外科2011年7月—2012年3月肺叶或全肺切除术患者216例,应用实时三维多普勒超声心动图连续对患者行术前RVEF测定。患者术前均行肺功能测定。分析RVEF与术后并发症的关系。结果:肺叶切除组RVEF45%的患者和RVEF≥45%的患者肺功能及术后并发症发生率差异均无统计学意义(P均0.05)。全肺切除组RVEF45%的患者和RVEF≥45%的患者肺功能差异无统计学意义(P0.05),而术后并发症发生率差异有统计学意义(P0.05)。结论:术前RVEF45%是全肺切除术后出现并发症的危险因素。

Correlation between Right Ventricular Ejection Fraction and Complications after Pulmonary Resection

Objective：To evaluate the correlation of right ventricular ejection fraction （RVEF） before operation and the risk of complications after pulmonary resection. Methods： A total of 216 patients undergoing pulmonary resections in Department of Thoracic Surgery of Shanghai Chest Hospital affiliated to Shanghai Jiaotong University from July 2011 to March 2012 were en- rolled. The preoperative RVEF in all patients was detected by real-time three-dimensional echocardiography. The preoperative puhnonary functions in all the patients were examined, and the relationships between RVEF and the complications were ana lyzed. Results： Neither lung functions nor the incidence of complications showed statistically significant differences between ca- ses with RVEF〈45% and cases with RVEF≥45% in pulmonary lobectomy patients （P〉0.05）. No statistically significant difference was found in lung functions between cases with RVEF〈45% and cases with RVEF≥45% in pneumonectomy pa tients （P〉0.05）, but there was a statistically significant difference in the incidence of postoperative complications between the two groups （P〈0.05）. Conclusions： RVEF〈45% before operation is a risk factor for complications after pulmonary resection.
