高尔基体蛋白73在肝细胞癌早期诊断及进展中的应用价值

目的 分析高尔基体蛋白73（GP73）并与甲胎蛋白（AFP）比较在肝细胞癌（HCC）中的临床诊断价值.方法 分别采用酶联免疫吸附试验和电化学发光免疫分析定量测定77例HCC、21例肝硬化、20例肝炎以及59例健康对照组血清GP73和AFP的水平,并进行分析比较.结果 HCC组血清GP73和AFP均显著高于肝硬化、肝炎及健康对照组（均为P＜0.05）;早期HCC组血清GP73显著高于肝硬化组（P＜0.05）,而血清AFP在两组中无明显差异（P＞0.05）.GP73对HCC的诊断敏感度明显高于AFP（85.7％vs 62.3％,P＜0.05）,两者联合检测可提高其诊断敏感度到90.9％.结论 GP73是一项很好的诊断和鉴别早期HCC的血清标志物,且可能对HCC进展的监测具有作用.AFP联合GP73检测可显著提高对HCC的临床诊断价值.

Serum golgi protein 73 for early diagnosis and surveillance of hepatocellular carcinoma

Objective To evaluate the diagnostic value of serum gholgi protein 73 （GP73） and alpha-fetoprotein （AFP） for early-stage hepatocellular carcinoma （HCC）.Methods The enzyme linked immunosorbent assay （ELISA） and eletrochemiluminescence immunoassay were employed to determine the level of serum GP73 and AFP in 77 patients with HCC,21 patients with liver cirrhosis,20 patients with hepatitis and 59 healthy subjects for further comparison and analysis.Results The levels of serum GP73 and AFP in HCC were significantly higher than those in cirrhosis,hepatitis and healthy controls （all P＜ 0.05）.Patients with early-stage HCC had a markedly higher level of GP73 than those with liver cirrhosis （P＜ 0.05）.However,there was no significant difference of AFP between these two groups （P＞0.05）.The diagnostic sensitivity of GP73 for HCC was significantly higher than that for AFP （85.7％ vs 62.3％,P＜ 0.05）.The combination detection was associated with an increased sensitivity （90.9％）.Conclusions GP73 is an ideal serum marker for the diagnosis of liver diseases and monitoring of disease progression.The combination detection with AFP effectively improves the diagnosis accuracy of HCC.
