液基薄层细胞学检查联合人乳头状瘤病毒基因分型检测在宫颈病变筛查中的应用

摘    要:目的:探讨液基薄层细胞学检查(ThinPrep cytologic test,TCT)联合人乳头状瘤病毒(human papillomavirus,HPV)基因分型检测在宫颈病变筛查中的应用价值。方法:选取同时行TCT检查、HPV基因分型检测以及病理组织学检查的宫颈病变疑似患者415例,比较不同方法的检测敏感度。结果:TCT检测出宫颈病变63例(15.18%),HPV基因分型检测出140例(33.73%),两者检测敏感度差异有统计学意义(P0.05);病理组织学检查阳性48例(11.57%),其中TCT联合HPV基因分型检测阳性率为89.86%(43/48),并与宫颈的病变程度呈正相关。HPV基因分型检测出的140阳性病例中,出现频率最高的为16型和52型(分别为27.3%和21.5%),其次为58型和18型(分别占18.4%和16.8%)。结论:TCT联合HPV基因分型检测能显著提高宫颈病变的检出率。

Abstract:Objective:To explore the application value of ThinPrep cytologic test (TCT) combined with human papillomavirus (HPV) genotyping in the screening of cervical cancer. Methods:A total of 445 suspected cervical lesion patients were selected, and detected by TCT test, HPV genotyping and histopathology test in parallel. The screening sensitivity among the different methods were compared. Results: There were 63 (15. 18% and 140 positive cases (33.73%) screened by TCT and HPV genotyping, respectively, with a significant difference in two methods (P〈0.05). There were 48 cases (11.57% ) were tested by histopathology test, in which the positive rate of cases by TCT combined with HPV genotyping was 89. 86%(43/48), and increased with the enhancement of cervical lesions. In the 140 HPV positive cases, the most majority were HPV 16 and HPV 52 (27.3% and 21.5%, respectively), next were HPV 58 and HPV 18 (18.4% and 16.8%, respectively). Conclusions.. TCT combined with HPV genotyping can significantly improve the detection rate of cervical lesions.
