云南省大理地区HIV感染者丙型肝炎病毒基因分型研究

摘    要:目的对大理地区HIV感染合并丙型肝炎患者进行丙型肝炎病毒(HCV)基因分型研究。方法采集患者血液,提取RNA,采用HCV特异性引物进行巢式RT-PCR扩增,PCR产物直接测序,采用生物信息学软件进行序列分析。结果在大理州人民医院共采集疑似丙型肝炎病人血清13份,HCV核酸检测4份(24~27号)阳性。序列分析显示4株病毒之间核苷酸同源性在80.4%~95.8%之间;24号与1b型HCV核苷酸同源性在98.6%以上,而其他基因型病毒核苷酸同源性在90.2%以下;25、27号与3b型HCV核苷酸同源性在92.1%~98.6%之间,而与其他基因型核苷酸同源性均在87.4%以下;26号与3a型HCV核苷酸同源性在94.4%~94.9%之间,而与其他基因型病毒核苷酸同源性在84.6%以下。遗传进化分析显示25、27号病例为3bHCV感染,26号病例为3aHCV感染,24号为1bHCV感染。结论大理地区HIV感染人群中存在HCV基因1和基因3两个基因型,3a、3b和1b3个基因亚型HCV感染。

Abstract:Objective To study the genotypes of the hepatitis C virus in patients coinfected with hepatitis C and HIV in Dali. Methods Serum samples were collected from patients suspected of having a coinfeetion with hepatitis C and HIV. RNA was extracted from the serum samples, which were analyzed using nested RT PCR amplification with HCV-specific primers. The RNA was sequenced, and sequence analysis was performed using bioinformatics software. Results Of 13 serum specimens from patients suspected of having a coinfection with hepatitis C and HIV in the First People's Hospital in Dali, 4 (24, 25, 26, and 27) were positive for HCV according to nested RT-PCR amplification with primers specific to the HCV C gene. The results of the nucleotide sequence analysis showed that the nucleotide similarity among the four strains varied from 80.4% to 95.8%. Sample 24 had the highest nucleotide similarity (98.6%) to genotype lb of HCV, but similarity to other genotypes of HCV was less than 90.2%. Samples 25 and 27 had nucleotide similarity of 92.1% to 98.6% to genotype 3b of HCV. These samples had nucleotide similarity of less than 87.4% to other genotypes of HCV. Sample 26 had nucleotide similarity of 94.4% to 94.9% to genotype 3a of HCV. This sample had nucleotide similarity of less than 84.6% to other genotypes of HCV. Results of phylogenetic analyses conducted with 493 nt of the HCV C gene suggested that samples 25 and 27 were the genotype 3b of HCV, sample 26 was genotype 3a, and sample 24 was genotype lb. Conclusion Genotypes 1 (subtype lb) and 3 (subtype 3a and 3b) of HCV are present among patients infected with HIV in the Dali region.
