腹腔镜胆囊切除术中胆囊动脉的解剖观察

目的探讨腹腔镜胆囊切除术中胆囊动脉的解剖变异及其临床意义。方法分析该科2005年8月～2006年3月78例腹腔镜胆囊切除术患者镜下胆囊动脉各型特点、走行变异及其胆囊壁入点分布规律等。结果78例腹腔镜胆囊切除术中,主干型胆囊动脉58例(74.4%),分支型胆囊动脉20例(25.6%);胆囊动脉在靠近胆囊壶腹部有入点者68支(66.7%),在靠近胆囊体部有胆囊动脉入点者32支(31.4%);胆囊动脉走行于胆囊三角内者67例(85.9%)。结论掌握腹腔镜胆囊切除术中胆囊动脉解剖变异,尤其是胆囊动脉走行和在胆囊壁常见入点的分布比例,对于减少腹腔镜胆囊切除术中出血及胆管、血管误损伤有重要意义。

Intraoperative anatomy of cystic artery during laparoscopic cholecystectomy

Objective To investigate the anatomy variation of cystic artery during laparoscopic cholecystectomy. Methods We studied the characteristic, variation and distribution at the gallbladder wall of every type of cystic artery under LC from August of 2005 to March of 2006. Result The main-trunk cystic artery accounted for 74.4%, and the mutiple-trunk cystic artery accounted for 25.6%;the sites of entrance to gallbladder ampullae accounted for 66.7%, and the sites of entrance to gallbladder body accounted for 31.4%;the cystic artery across Calot triangle accounted for 85.9%. Conclusion Mastering the anatomy variation of cystic artery, especially the variation of extending route and distribution at the gallbladder wall, has great value in decreasing bleeding and important structures injury during laparoscopic cholecystectomy.
