变质甘蔗中毒临床随访和预防

目的 为了探讨霉变甘蔗中毒的临床特点、病理改变及预防措施.方法 对18例重症霉变甘蔗中毒进行临床分析、远期随访、病原学检查、动物模型及病理检查.结果 临床特点:均以吐泻为首发症状、明显无力、步态不稳、视物不清,迅速出现嗜睡、昏迷、惊厥、颈项强直、扭转痉挛.以食剩甘蔗培养物喂哺小白鼠后产生类似症状,患儿尸检及病鼠脑组织可见广泛性充血、水肿、变性、坏死.结论 节菱孢霉菌为霉变甘蔗中毒主要致病菌.胃肠道、神经、椎体外系症状为主要临床特点.甘蔗采购、贮存、质检及普及有关知识是预防的关键.

Clinical Analysis, Follow-up and prevention of the Mouldy Sugarcane Poisoning

ve To discuss the clinical features, pathologic changes and prevention of the mouldy sugarcane poisining. Methods Clinical analysis, long term follow-up, pathogenic examination, building of animal model and pathologic examination of eighteen severe mouldy sugarcane poisoning cases. Results Clinical features: Vomiting and diarrhea are the initial syptoms of all cases. The patients have obvious weakness, unstable gait, vague sight and so on separately. Such syptoms as lethargy, coma, convulsion, cervical tetanus and twist spasm occur quickly. And We can get the similar syptoms by feeding six mice with the residual mouldy sugarcane culture with the autopsy both of the patients and mices, we can find extensive congestion, edema, degeneration and necrosis of the brain tissue. Conclusions Arthrinium Saccharicold is the main pathogen of the mouldy sugarcane poisoning. The syptoms of the gastroenteric tract, nerve system and extrapyramidal system are the main clinical features of this disease. The key measures of the prevention of this disease is to be more careful during the purchase, storage and quality examination of sugarcanes. The knowlege about sugarcane should be popularuized widely.
