加速康复外科在后腹腔镜肾癌根治术中的应用

目的加速康复外科(fast tract surgery,FTS)理念在外科科室中的应用逐渐普及。文中旨在应用FTS的理念观察泌尿外科后腹腔镜肾癌根治术患者临床应用的可行性及优势。方法回顾性分析42例行后腹腔镜肾癌根治术的患者资料,其中采用围手术期FTS治疗20例(FTS组),传统方法治疗22例(对照组)。分析2组患者术后恢复、术后住院时间、住院费用以及应激指标等情况。结果与对照组比较,FTS组患者术后通气时间、术后进食时间、术后下床时间、术后住院天数及住院费用均显著减少(P<0.01);FTS组术后1 d血糖及胰岛素抵抗水平较对照组显著降低(P<0.05)。结论 FTS在泌尿外科后腹腔镜肾癌根治术中的应用具有可行性且更具优势。

Clinical application of fast track surgery in retroperitoneal laparoscopic radical nephrectomy

Objective The theory of fast tract surgery(FTS) is gaining a wide application in surgery.This study aimed to observe the feasibility and advantages of the clinical application of retroperitoneal laparoscopic radical nephrectomy based on the theory of FTS.Methods We retrospectively analyzed 42 cases of retroperitoneal laparoscopic radical nephrectomy,20 treated by FTS in the perioperative period(FTS group),and the other 22 by the conventional method(control group).We compared the two groups in their postoperative recovery time and bed days,hospitalization costs,and stress indexes.Results Compared with the controls,the FTS group showed a significant reduction in postoperative ventilation time,postoperative feeding time,postoperative ambulation time,postoperative hospital stay and hospitalization costs(P<0.01),as well as markedly lower levels of blood glucose and insulin resistance(P<0.05).Conclusion Fast track surgery is feasible and effective in urological retroperitoneal laparoscopic radical nephrectomy.
