螺旋CT多期扫描技术对胰腺癌的诊断价值

目的：探讨螺旋CT多期扫描对胰腺癌的诊断价值。方法：搜集行SCT动脉期、胰腺期、肝脏期三期扫描的胰腺肿块75例，均经手术病理证实，其中胰腺癌53例，局限性胰腺炎22例，比较肿瘤与炎症在SCT增强扫描各期中的表现。结果：本组胰腺癌53例，其中胰头癌33例，胰颈癌4例、胰体癌14例，胰尾癌2例。局限性胰腺炎22例，头、颈、体、尾分别为17例、2例、2例、1例。增强扫描，动脉期胰腺癌低密度43例，等密度10例，胰腺炎低密度2例，等密度20例；胰腺期胰腺癌低密度51例，等密度2例，胰腺炎低密度1例，等密度21例；肝脏期胰腺癌低密度42例，等密度11例；胰腺炎22例均为等密度。结论：胰腺螺旋CT动脉期、胰腺期、肝脏期三期增强扫描在胰腺癌诊断中具有非常重要的临床价值。

Pancreatic carcinoma:CT findings and technique of multi-phase spiral CT

Objective:To study the protocols of multi-phase spiral CT for the evaluation of pancreatic carcinoma.Methods:Seventy five patients with pancreatic mass underwent multi-phase spiral CT. All were scanned in arterial, pancreatic and hepatic phase. All were confirmed by surgical and pathological findings:there are 53 cases with pancreatic carcinoma, and 22 cases with localized pancreatitis. Multi-phase spiral CT scanning after administration of contrast material were underwent in all the cases with pancreatic carcinoma or localized pancrtitis, which is confirmed by surgical and pathological findings. The CT findings of pancreatic carcinoma and localized pancreatitis were compared in three phase CT images.Results:There are 33 cases with pancreatic carcinoma in the head of pancreas, 4 cases in the neck of pancreas,14 cases in the body of pancreas,2 cases in the tail of pancreas. While 17 cases with localized pancreatitis in the head of pancreas, 2 cases in the neck of pancreas, 2 cases in the body of pancreas, 1 cases in the tail of pancreas. The helical CT appearances of 75 patients were as follows: in arterial phase, the lesion of pancreatic carcinoma appeared as hypointense (43/53) and isointense (10/53), while localized pancreatitis hypointense (2/22) and isointense (20/22). In pancreatic phase, the lesion of pancreatic carcinoma appeared as hypointense (51/53) and isointense (2/53) while localized pancreatitis hypointense (1/22) and isointense (21/22).In hepatic phase, the lesion of localized pancreatitis isointense (22/22).Conclusion:Multi-phase spiral CT scanning is valuable in detecting pancreatic carcinoma.
