MRI对糖尿病足部神经血管病变的诊断价值

目的研究MRI对糖尿病足部神经血管病变的诊断价值。方法对14例有足部神经血管病变的糖尿病患者行MRI、X-线平片、同位素骨扫描。结果14例患者中10例MRI诊断为血管神经病变。X-线平片无法显示早期骨髓炎，仅在晚期表现为骨质破坏。同位素骨扫描鉴别软组织及骨髓变化较困难。结论MRI可显示细微解剖结构，诊断糖尿病引起的血管、神经性病变较X－线平片及放射性同位素骨扫描敏感，特异性高，是诊断糖尿病足部神经血管病变的有效方法。

Evaluation of MRI in Diabetic Foot Neuroarthropathy

Objective To evaluate the value of MRI in diagnosing diabetic foot neurovascular changes. Methods 14 diabetic patients with clinical suspiction of osteomyelitis, nonhealing foot ulcer or soft tissue infection of the foot were examined by MRI and plain radio- graphy and radioisotope scan. Results Clinnical or surgical/pathological confirmation of the MRI diagnosis was obtain in all but four. Plain radiographs cannt be shown in imaging osteomyelitis directly. Conclusion MRI has sensitivity and accuracy compared with plain radiogyaphs in the diagnosis of neuroarthopathy from osteomyelitis in diabetic foot iofections. MRI is a valuable adjunct in the evaluation of the diabetic foot. MRI bas proven paticluarly helpful in differentiating neuroarthropathy form osteomylitis.
