眼内异物摘出术中巩膜表面解剖学标志定位法

摘    要:目的 探讨眼球后段异物外路(后径)摘出术中巩膜表面径线定位新方法,提高眼后段异物摘出手术效果。方法 以离体及原位眼球巩膜表面的解剖学标志为根据,对95例眼球后段异物,按术前X线摄片所示的眼内不同位置,术中选用眼外肌、涡静脉或捷状后长动脉径线定位切口摘出异物。结果 本法定位的95例与20例角膜缘缝线标志法的术后效果进行对照分析,体会到,前者一次性切口异物摘出成功率、脱盲脱残率及并发症发生率等均优于后者。结论 术中巩膜解剖学标志定位,方法简单、创伤小、准确性高,可避免术中牵拉旋转眼球所造成的径线位误差,是外路异物摘出术中一种较为可靠的径线定位新方法。

Abstract:Objective To explore a new localization method of foreign body in the posterior segment according to the marks on sclera surface, in order to improve the effect of extraction of foreign body in the ocular posteriror segment. Methods According to the position of foreign body on the Xray, 95 cases of intraocular foreign body were operated by means of the marks of vortex vein and posterior ciliary artery on the sclera surface and compared with the 20 cases of intraocular foreign body operated by the marks of corneal limbus. Results In this series, the rate of postoperative success is higher and the rate of complication is lower than that of control group. In addition, this method is simple, has little damage to the eye and can avoid the location error resulted from dragging eye. Conclusion Extraction of metal intraocular foreign body according to the marks on sclera surface is a dependable and exact method.
